中文摘要 | 在本院外科於近二年內有十六例手術後殘留肝內結石症之病人接受以5%碳酸氫鈉溶液來當為溶石液之治療經驗其方法是先以一細管引入T-型管內且使其管子之末端接近胆石為原則或T-型管拔出再以Nelaton管子代入再以此液200西西由此細管子(或Netaton管子)慢慢滴入,在治療時須使病人上身部放低,使此溶液能進入肝內,故能與胆石接觸完全及較長時間為目的。在治療前及治療中均每二週視胆管照影術來比較前後治療成績。 我們以此法治療之結果成功者布6例;有改善者(指胆石數目減少或體積縮小者)有6例,但其中二例在九個月及十個月後症狀再發,接受第二次手術;治療失敗者有4例,但其中有一例至今仍無症狀發生。 此溶液在試管中做熔石實驗是有效的,應用在臨床上也是相當簡單且經濟有效,不但可供使用在肝內結石亦可應用在殘留總胆管結石之病患,故對胆管方面有殘留之胆石者宜嗜試此法浸洗,若失敗或胆道鏡也無法取石成功者才施行手術探討治療。 |
英文摘要 | Before the treatment, 5% sodium bicarbonate solution had a higher disintegration ability proved by high bilirubin level and more calcium ion escaped from calcium bilirubinate stones bathed in this solution in our experimental study. Sixteen patients with retained intraphepatic stones were treated by dissolution with 5% sodium bicarbonate solution intracholedochally. Six patients had completely cured with this method, 6 patients had improved for diminishing the size and number of stones and the other 4 were failed. There was only one case with complications of nausea and vomiting after perfusion, but subsided spontaneously without treatment. Practically, this nonsurgical treatment for retained intrahepatic stones by dissolution is a safe and simple method and also available for retained common bile duct calculi. Therefore, this method should be used routinely before re-exploration for retained calculi. |
