| 中文摘要 | 抗藥性金黃色葡萄球菌(methicillin-resistant Staphylococcus aureus, MRSA)會造成臨床上嚴重的感染,且對於多種抗生素都有抗藥性,往往造成治療上的困難。而傳統的鑑定方法又較費時,因此抗藥性金黃色葡萄球菌(MRSA)與非抗藥性金黃色葡萄球菌(methicillin-sensitive Staphylococcus aureus, MSSA)的快速鑑定對於感染的控制與治療就顯得重要。爲求快速鑑定MRSA與MSSA,本研究嘗試利用Luminex xMAP的系統建立一套分生診斷方法。研究結果發現,以Huletsky等人針對orfX的特定序列所設計forward primer Xsau325,及以SCCmec element爲reverse primer進行聚合酶連鎖反應(PCR)後進行電泳分析,雖可有效區分金黃色與非金黃色葡萄球菌,但無法完全區分MRSA與MSSA。但如以兩種偵測MRSA的特異性探針(XsauB5或 XsauB8),在Luminax儀器雜合反應所得到的螢光強度進行判讀,就可得到MRSA螢光強度明顯高於MSSA的螢光值,其中XsauB8探針所得結果特異性較高。因此,從研究結果得知,以XsauB8探針結合Luminex xMAP系統,可快速有效的檢測並區分MRSA與MSSA,進而協助正確的診斷與治療,避免MRSA的傳染,有效降低醫療成本。 |
| 英文摘要 | Objective: Methicillin resistant Staphylococcus aureus (MRSA) strains are predominantly implicated in serious infections. Because MRSA strains are resistant to various antibiotics, treatment options are limited consequently and technology for rapid detection is crucial for both curative measures and infection control. This study established a rapid molecular diagnostic technology based on Luminex xMAP system for the detection and differentiation of MRSA and methicillin-sensitive S. aureus (MSSA). With the use of lasers which excite the internal dyes to identify each microsphere probe and measure the fluorescence of PCR products hybridized with the specific probes on the beads, Luminex xMAP technology is a platform for simultaneous detections of specific targets. The results indicated that the PCR reaction with the use of S. aureus orfX specific forward primer (Xsau325) and SCCmec element based reverse primer could differentiate Staphylococcus spp. from non-staphylococcus. However there were still ambiguities in the distinction of MRSA and MSSA. To improve the specificity of PCR reaction, the amplification products were hybridized with specific probes with fluorochrome labeled and analyzed by Luminex system to further differentiate MRSA from MSSA. With the two specific probes, XsauB5 and XsauB8, a higher fluorescent value was obtained in MRSA than in MSSA to differentiate these two Staphylococci explicitly. Additionally, XsauB8 worked better for MRSA detection as the difference between MRSA and MSSA were greater. A brief conclusion can be made that the combination of XsauB8 probe and Luminex system is an efficient tool to differentiate MRSA and MSSA with high accuracy. |
