| 中文摘要 | 本文旨在探討臺灣長照 3.0 政策脈絡下,照顧服務員於安寧療護與居家照護體系中 的專業角色定位與發展契機。隨著高齡化與慢性病盛行,長照人力短缺與角色定位模糊 已成為政策推動的關鍵挑戰。透過政策與國內外文獻之整合分析,結果顯示:照顧服務 員在接受系統化訓練與適切照護授權後,於日常生活支持、病況觀察、情緒支持及輔助 療法運用等面向可展現重要功能,並有助於強化安寧照護之連續性與品質。然而,目前 制度在進階訓練、專業督導及激勵措施上仍存不足,限制其專業角色之深化與實質貢獻。 為提升其在安寧照護團隊中的專業能見度與角色貢獻,建議於長照 3.0 架構中明確界定 角色、建置專業分級制度,並強化安寧導向教育與臨床支持。本文結果可供政策制定者 與教育者作為推動安寧理念融入長照服務、建構永續且以人為本之照護模式的重要參據。 |
| 英文摘要 | This paper explores the professional role construction and developmental opportunities for care workers within hospice and home care settings under the framework of Taiwan’s LongTerm Care 3.0 policy. Amidst a rapidly aging population and the rising prevalence of chronic diseases, workforce shortages and role ambiguity have emerged as critical impediments to effective policy implementation. Drawing upon a comprehensive review of policy documents and relevant literature, the results indicate that with specialized training and appropriate delegation, care workers can perform pivotal functions—ranging from daily living support and symptom observation to emotional companionship and complementary therapy—thereby significantly enhancing the continuity and quality of hospice care. However, the current infrastructure restricts these professionals from fully realizing their value due to insufficient advanced training, a lack of professional supervision, and inadequate incentive mechanisms. To establish care workers as indispensable members of the interdisciplinary hospice team, it is imperative to strengthen role delineation, implement a professional grading system, and reinforce hospice-oriented education within the Long-Term Care 3.0 architecture. These findings provide a robust reference for policymakers and educators aiming to integrate hospice philosophies into the long-term care system and foster a sustainable, person-centered care model. |
