| 中文摘要 | 動靜脈瘻管是血液透析病人生命線,當發生狹窄或阻塞時,會增加醫療成本, 影響透析品質並增加病人身心負擔。本單位 2021 年動靜脈瘻管平均月阻塞率為 3.0%,2022 年升至 3.7%,造成 4 名病人需置放永久性導管、1 名進行瘻管重建,故 引發改善動機。專案期間 2023/2/1~2023/11/30,經現況分析確認問題為病人自我照 護不當、水分控制不佳導致低血壓、穿刺部位未輪替、瘻管評估不確實、作業規範 未更新、教育訓練不足,以及缺乏合適工具與設備維護。對此,專案小組進行規範 修訂、加強護理人員教育訓練與團體衛教,並提供多元衛教工具及設備維護,以提 升護理人員評估落實度與病人自我照護能力。專案執行後,瘻管阻塞率降至 0.6%, 顯示改善措施具實質成效,並可作為臨床照護參考。 |
| 英文摘要 | In 2021, the average monthly obstruction rate of vascular access in our unit was 3.0%. In 2022, it rose to 3.7%, resulting in 4 patients requiring permanent indwelling catheters and 1 patient undergoing vascular access reconstruction, which affected the quality of dialysis and increased the physical and mental burden on patients, thus triggering the motivation for improvement. After identifying the problems through the project, the main reasons were found to be poor self-care by patients, poor water control leading to hypotension, lack of rotation at puncture sites, inaccurate access assessment, outdated operation guidelines, insufficient education and training, as well as inadequate maintenance of appropriate tools and equipment. In response, the project team carried out guideline revisions, strengthened the teaching and training of nursing staff, provided diverse teaching tools and equipment maintenance, and enhanced the nurses’ assessment implementation as well as patients’ self-care abilities. After the implementation of the project, the obstruction rate of vascular access decreased to 0.6%, showing that the improvement measures were indeed effective, and could serve as a clinical nursing reference. |
