| 中文摘要 | 本文敘述一位因車禍導致創傷性主動脈損傷、面臨死亡焦慮個案,在臨床上較罕見,故引發筆者探討之動機。筆者於2023年3月22日18:40-21:40護理期間,藉由直接照護、身體評估、訪談、觀察等方式收集資料,進行生理、心理、社會、靈性評估,確立健康問題有:潛在危險性心臟組織灌流減少、急性疼痛、死亡焦慮。個案面臨突如其來的車禍意外,一時難以接受需緊急手術及病危住院的噩耗,並產生死亡焦慮情形,急診團隊在照護期間,以維持穩定的血流動力學、給予足夠的氧和,為優先照護目標,並以關懷、接納、傾聽及引導式溝通,協助釐清個案內心焦慮主因,鼓勵抒發感受,結合醫療團隊向個案及家屬解釋後續治療的必要性及增加病人對疾病的認知,以澄清個案的疑問及緩解焦慮情形及建立良好的護病關係。 |
| 英文摘要 | This article describes the emergency nursing experience of a patient who suffered traumatic aortic injury due to a car accident and faced death anxiety. This is a relatively rare case in clinical practice, which motivated the author to explore further. During the nursing care period of 18:40 to 21:40 on March 22, 2023, the author collected data through direct care, physical assessment, interviews, and observation; conducting physiological, psychological, social, and spiritual assessments. Hence identified health issues including potential risk of decreased cardiac tissue perfusion, acute pain, and death anxiety. The case was facing a sudden shock after the car accident, found it difficult to accept, also needed emergency surgery and hospitalization, consequently evolved death anxiety. During the care, the emergency team prioritized maintaining stable hemodynamics and providing adequate oxygenation. The emergency team applied care, acceptance, listening, and guided communication to help clarify the main cause of the patient's anxiety, encouraged the patient to express his feelings; in collaboration with the medical team, explained the necessity of subsequent treatment to the patient and his family. This approach aimed to enhance the patient's understanding of the illness, alleviate anxiety, and establish a good nurse-patient relationship. It is hoped that this nursing experience will serve as a reference for the clinical staff in caring for similar cases in the future. |
