| 中文摘要 | 本研究旨在探討高齡失能者主要家庭照顧者之照顧需求與家庭功能,並分析其影響因素。採橫斷式量化研究設計,於2022年4月22日至8月1日間,針對南部某榮民區域醫院收案78位家庭照顧者,使用結構式問卷收集基本資料、照顧需求與家庭功能量表。結果顯示照顧者平均年齡58.44±13.69歲,多為女性(57.7%),71.8%有佛道教信仰;與被照顧者關係最多為配偶(35.9%),每日平均照顧時數為16.59±8.12小時,家庭型態以小家庭(59.0%)為主。高齡失能者平均77.83±9.48歲,19.2%有失智症,慢性病數平均3.54項,以高血壓與糖尿病最常見。照顧者自評家庭功能-40~136之量表,平均得分93.73±22.84,屬中等以上水準。照顧者以生理需求最迫切,尤以「充足睡眠」需求最高。照顧者需求與家庭結構、性別、宗教信仰及自覺健康狀況等因素有顯著關聯:小家庭者對「實質支持」與「資訊支持」需求較高,女性在「資訊支持」需求較明顯,有宗教信仰及自評健康愈佳者在家庭功能表現顯著較高,年長的照顧者具較佳問題解決能力與較低衝突傾向。多元迴歸分析指出宗教信仰與健康自評為正向預測因子,高齡者疾病數、子女數及照顧者教育程度為負向因子,解釋力達27.5%。建議強化小家庭與高依賴個案之支持系統,並提供充足的資訊與情緒支持,以提升家庭照護品質與功能表現。 |
| 英文摘要 | This study explored the care requirements and family function of primary family caregivers of disabled older adults in Taiwan. A cross-sectional quantitative design was applied, and data were collected via structured questionnaires from 78 caregivers at a southern regional Veterans hospital between April 22 and August 1, 2022. The questionnaire included demographics, care requirements, and family function scale. An average age of caregivers was 58.44±13.69 years, with a majority being female (57.7%) and 71.8% identifying with Buddhist or Taoist beliefs. Most caregivers were spouses of care recipients (35.9%), with an average daily caregiving time of 16.59±8.12 hours and primarily in nuclear family settings (59.0%). Care recipients averaged 77.83±9.48 years old, with 19.2% diagnosed with dementia and an average of 3.54 chronic illnesses, most commonly hypertension and diabetes. Self-rated family functioning, measured on a scale from 40 to 136, had an average score of 93.73±22.84, indicating a moderate to high level. Physiological needs, especially adequate sleep, were reported as the most urgent care requirements. Significant associations were found between caregiver needs and factors such as family structure, gender, religious belief, and perceived health status. Specifically, caregivers in nuclear families expressed stronger needs for tangible and informational support, women showed higher informational support needs, and those with religious beliefs and better perceived health had significantly higher levels of family functioning. Older caregivers demonstrated better problem-solving abilities and lower conflict tendencies. Multiple regression analysis revealed that religious belief and perceived health were positive predictors of family functioning, while the number of illnesses in the care recipient, number of children, and caregiver education level were negative predictors, explaining 27.5% of the variance. It is recommended to strengthen support systems for nuclear families and high-dependency cases, and to provide adequate informational and emotional support to enhance caregiving quality and family function. |
