| 中文摘要 | 本文描述一位妊娠37週又3天的經產婦,因確診感染COVID-19而接受剖腹生產。產後入住負壓隔離病房,無法進行親子同室及母乳哺餵,使其感到極度焦慮與不安,並對自身能否成功哺乳及履行母職產生強烈疑慮與信心不足。護理期間自2022年9月5日至9月10日,評估後發現主要護理健康問題包括:低效性母乳哺餵、對新冠疾病病程的焦慮、情境性低自尊。本案例運用Swanson關懷理論作為護理指導原則,透過「了解」、「在旁陪伴」、「為她做某些事」、「使她能夠」及「維持信念」等五大關懷核心進行護理介入。此外,結合多元通訊軟體、視訊教學及針對COVID-19的衛教手冊,提供個案全方位的照護支持。經過入院至出院及居家護理的關懷介入,個案從最初的信心存疑轉變為成功建立母職信心,並能持續哺乳。期望藉由本案例的照護經驗,提供因傳染性疾病影響的孕產家庭更多支持與幫助,協助其順利過渡至親職角色,進而提升母職信心與新生兒健康發展,作為臨床護理實務的參考。 |
| 英文摘要 | This article describes a multiparous woman at 37 weeks and 3 days of gestation who underwent a cesarean section after being diagnosed with COVID-19. She was admitted to a negative-pressure isolation ward postnatal, preventing room-in with her newborn and breastfeeding. As a result, she experienced extreme anxiety and distress, as well as significant doubts and lack of confidence in her ability to breastfeed and fulfill her maternal role. The nursing period was from September 5 to 10, 2022. After evaluation, the main nursing health issues identified include: (1)ineffective breastfeeding,(2)anxiety regarding the duration of COVID-19, and (3)situational low self-esteem. This case applied Swanson's Theory of Caring as a framework for nursing interventions, incorporating the five core caring principles: "knowing," "being with," "doing for," "enabling," and "maintaining belief." In addition, various communication software, video-based education, and an instructional manual tailored for COVID-19 were utilized to provide comprehensive care and support. Through continuous nursing interventions from hospitalization to discharge and home care, the patient transitioned from initial uncertainty to successfully establishing maternal confidence and sustaining breastfeeding. Hopefully this nursing experience will provide more support and assistance to pregnant and postpartum families affected by infectious diseases, assist them in smooth transitioning to parental roles, strengthen maternal confidence and the healthy development of newborns, and hence serve as a reference for clinical nursing practice. |
