| 中文摘要 | 本綜論探討以教練式教學策略推動精神科實證照護的實務歷程與成效。實證護理強調運用最佳研究證據、結合臨床經驗與病人偏好,以提升照護品質與安全。然而,實證理念落實於臨床仍面臨多重挑戰,尤其在精神科臨床情境中,常見障礙包括知能落差、資源不足與組織文化抗拒等。臺北市立聯合醫院松德院區自2011年推動實證教育訓練,2019年起導入教練式策略,系統性規劃三階段歷程:實證概念啟蒙期、實證專家培育期與單位種子人員培育期,逐步建構院內實證推動體系。教練式策略四大推動要素:明確策略設計、階段性執行步驟、團隊角色分工及時間軸規劃,並剖析其在精神科臨床的適用性與挑戰因應。實施成果顯示,教練式策略促進護理人員實證知識、批判性思維與臨床轉譯能力之提升,並有效應用照護主題音樂治療、運動、園藝、生活技能訓練等。教練式策略不僅促進精神科實證照護之落地與深化,更助於臨床文化轉變與護理專業認同建立,建議未來納入制度化與科技化之培育模式,以擴大其永續發展潛能。 |
| 英文摘要 | This review explores the practical process and outcomes of implementing evidence-based psychiatric care through a coaching strategy. Evidence-based nursing emphasizes integrating the best research evidence with clinical expertise and patient preferences to improve care quality and safety. However, translating evidence-based concepts into clinical practice remains challenging, particularly in psychiatric settings, where common barriers include knowledge gaps, insufficient resources, and resistance within organizational culture. Since 2011, Taipei City Hospital Songde Branch has promoted evidence-based education and training, and in 2019, it introduced a coaching strategy. This approach systematically progressed through three phases: the initiation of evidence-based concepts, the cultivation of evidence-based experts, and the training of unit-based seed personnel, gradually establishing an internal framework for evidence-based promotion. The coaching strategy was driven by four key elements: clearly defined strategic design, phased implementation steps, delineation of team roles, and timeline planning. Its applicability and corresponding challenges in psychiatric clinical practice were also examined. Outcomes indicated that the coaching strategy enhanced nurses' evidence-based knowledge, critical thinking, and clinical translation skills, leading to effective applications in care topics, such as music therapy, exercise, horticultural therapy, and daily living skills training. The strategy facilitated the integration and deepening of evidence-based psychiatric care, but also contributed to cultural transformation within clinical settings and the strengthening of nursing professional identity. Future recommendations include incorporating institutionalized and technology-enhanced training models to maximize its potential for sustainable development. |
