| 中文摘要 | 本文為運用自我效能理論於一位缺乏病識感思覺失調症個案成效探討,個案自2021年11月起開始出現幻聽、被害妄想、被監視感,2022年5月被家人帶至精神科首次住院。筆者照護期間由2022年5月4日至2022年6月25日,藉由實際觀察、會談、病歷閱讀等方式,採用生理、心理、智能、社會、靈性五大層面之精神衛生護理評估進行資料收集,發現個案主要的健康問題有無效性健康維護能力、感覺及知覺紊亂。護理期間透過Bandura自我效能理論,與個案建立信任關係、提供疾病及藥物知識、教導轉移注意力的方法、加強因應幻聽及妄想的能力,提升病識感並能夠配合醫療活動,最後可學習適應與幻聽及妄想共存。期望藉此護理經驗分享,使精神科護理人員往後照護缺乏病識感之思覺失調症個案時,可運用自我效能理論擬定合適照護計畫,提升其自我健康維護能力及減輕精神症狀干擾。 |
| 英文摘要 | This study explored the effectiveness of applying self-efficacy theory in the care of a patient with schizophrenia poor insight. The patient began experiencing auditory hallucinations, persecutory delusions, and a sense of being watched in November 2021 and was first admitted to a psychiatric ward in May 2022 at the insistence of family members. During the nursing period from May 4 to June 25, 2022, data were collected through direct observations, interviews, and medical record reviews. A mental health assessment was conducted based on five key aspects: physiological, psychological, cognitive, social, and spiritual. The primary health issues identified included ineffective health maintenance and sensory-perceptual disturbances. Nursing interventions were implemented on the basis of Bandura's self-efficacy theory, emphasizing the establishment of a trusting relationship with the patient, comprehensive patient education on the illness and medication, the application of effective distraction techniques, and the reinforcement of copying strategies for managing hallucinations and delusions. These approaches improved the patient's insight into their condition and improved their adherence to treatment, ultimately enabling them to adapt to living with persistent hallucinations and delusions. This nursing experience highlights the potential of applying self-efficacy theory to guide the development of effective care strategies for patients with schizophrenia and poor insight, thereby enhancing their self-care abilities and reducing the impact of psychiatric symptoms. |
