| 中文摘要 | 背景:失智症為老年人常見疾病之一,照顧失智症個案充滿著挑戰,故家庭主要照顧者之照顧負荷與生活品質需要被關注。目的:探討失智症患者家庭主要照顧者之照顧負荷、憂鬱與健康生活品質及其相關性。方法:採橫斷式相關性研究設計,以立意取樣,針對雙北市八家社區機構失智症患者之家庭主要照顧者為研究對象。研究工具包含照顧者負荷、憂鬱與健康生活品質量表,資料分析以t檢定、單因子變異數分析主要照顧者之背景變項與照顧者負荷、健康相關生活品質之差異,以皮爾森相關分析照顧者照顧負荷、憂鬱、健康生活品質之相關性,並以回歸分析找出生活品質影響因素。結果:共101位失智個案及家庭主要照顧者參與,主要照顧者照顧負荷、憂鬱、健康生活品質平均得分分別為30.90±11.00分、17.03±8.41分及62.66±18.25分。64.4%主要照顧者出現憂鬱危險傾向。照顧者負荷與診斷失智症、失智嚴重度及精神行為症狀呈顯著相關性(p<0.05)。而失智症個案性別、失智嚴重度及精神行為症狀與照顧者健康生活品質達顯著相關性(p<0.05)。主要照顧者自覺健康狀況與照顧者負荷(p<0.05)及健康相關生活品質達顯著相關性(p<0.05)。主要照顧者自覺健康狀態、兼照顧其他家人、憂鬱及照顧者負荷為預測主要照顧者健康生活品質之變項,可解釋主要照顧者健康生活品質66%之總變異量(F=17.177,p<0.001)。結論:失智症患者失智嚴重度和精神行為症狀為影響照顧者照顧負荷與健康生活品質之因素,主要照顧者之照顧負荷與憂鬱、健康生活品質密切相關,主要照顧者自覺健康狀態、兼照顧其他家人、憂鬱及照顧者負荷為影響照顧者健康生活品質之重要預測因子。 |
| 英文摘要 | Background: Dementia is one of the common diseases among elderly individuals. Dementia care is complex and challenging. Therefore, the caregiver burden and quality of life of family caregivers need to be understood. Objectives: The study aims to explore the relationship between care burden, depression, and healthy quality of life in family caregivers of dementia Methods: This is a cross-sectional study design, using purposive sampling to select primary caregivers of dementia patients from eight community institutions in the northern region. Research instruments included the Caregiver Burden Scale, Depression Scale, and Health-Related Quality of Life Scale. The study used t-tests and one-way ANOVA to analyze the differences in caregiver background variables, caregiving burden, and health quality of life. Pearson's correlation analysis was used to explore the relationships among caregiver burden, depression, and health quality of life. Additionally, regression analysis was used to identify factors influencing quality of life. Results: A total of 101 dementia patients and their primary caregivers participated. The mean scores for caregiver burden, depression, and health quality of life were 30.90±11.00, 17.03±8.41, and 62.66±18.25, respectively. 64.4% of primary caregivers had a risk of depression. Caregiver burden was significantly correlated with patient's diagnosis, severity of dementia, and behavioral and psychological symptoms of dementia, BPSD (p<0.05). Patient gender, severity of dementia, and BPSD were significantly associated with caregiver health quality of life (p<0.05). Primary caregivers' perceived health status was significantly correlated with caregiver burden (p<0.05) and health quality of life (p<0.05). The perceived health status of primary caregivers, taking care of other family members, depression, and caregiver burden are important variables that predict the health quality of life of the primary caregiver, explaining 66% of the total variance (F=17.177, p<0.001). Conclusions: The severity of dementia and behavioral symptoms in dementia patients are factors influencing caregiver burden and health quality of life. Caregiver burden is closely related to depression and health-related quality of life. The perceived health status of primary caregivers, taking care of other family members, depression, and caregiver burden are significant predictive factors affecting the health quality of life of the primary caregiver. |
