| 中文摘要 | 本文描述照顧一位 72 歲男性初次罹患急性心肌梗塞個案之加護護理經驗, 個案因初次罹患心肌梗塞,對此疾病及加護病房環境不了解,導致個案產生焦 慮。護理期間為 2022 年 10 月 15 日至 10 月 17 日,運用 Gorden 十一項功能性 健康型態評估工具,採觀察、會談、身體評估及直接照護等方式收集資料,發 現個案有心輸出量減少、焦慮及知識缺失健康問題,照顧過程中密切監測生命 徵象以及早發現個案有無胸悶痛或呼吸喘加劇情形,協助臥床休息以降低心肌 耗氧量;運用數位產品發送電子衛教單張、自我介紹並建立良好的治療性關 係,在執行任何治療時,適時給予關懷及鼓勵表達內心感受,教導如何使用正 念技巧及塗抹精油,以緩解焦慮;提供個案保健心血管的健康飲食及運動計 畫,使個案可改變生活型態,以增進健康。建議可開設心靈相關課程,例:如 何引導個案進入正念冥想,緩解焦慮情緒,希望藉由此護理經驗,以利日後臨 床護理師照顧此類個案之參考。 |
| 英文摘要 | This study describes the experience of providing intensive nursing care to a 72-yearold male patient newly diagnosed with acute myocardial infarction. The patient developed anxiety due to unknown illness and the intensive care unit environment. Care was provided from October 15 to 17, 2022. A valuation framework based on Gordon's 11 Functional Health Assessment was applied, data were collected through observations, interviews, physical examination, and direct nursing care. The nursing proceeding identified the patient’s health issues include declined cardiac output, anxiety, and ignorance. During the period of nursing care, patient’s vital signs were intensively monitored for early detection of chest pain or exacerbated shortness of breath. Bed rest assistance was provided in order to lower myocardial oxygen consumption. Digital health leaflets were distributed and self-introduction has established a good therapeutic relationship. When the author performs treatment, provide timely care and encourage patient to express inner feelings, teach mindfulness techniques and apply essential oils to relieve anxiety. Additionally, a healthy diet and exercise plan was developed to help the patient maintain good cardiovascular health and hence change his lifestyle. It is recommended to establish spiritual courses such as: how to guide patients into mindfulness meditation and relieve anxiety. It is also hoped that this nursing experience can be used as a reference for clinical nurses to care for such cases in the future. |
