| 中文摘要 | 本文旨照顧一位食道癌病人接受空腸造瘻手術,面對營養照護及術後空腸 造瘻口照護之護理經驗。護理期間自 2022 年 7 月 26 日至 8 月 23 日,以 Gordon 11 項功能性健康型態評估,藉由觀察、直接照顧、身體評估及與個案互動等方 式,透過評估結果彙整出個案之健康問題有營養失衡:少於身體所需、疼痛、 皮膚完整性受損和焦慮等四項。在整個護理過程發現個案嚴重營養缺乏、手術 後空腸造瘻管灌食滲漏、術後疼痛及再次手術感到焦慮等生理及心理需求,透 過跨領域專業團隊合作,與醫師、營養師以及造口護理師等共同協助改善個案 健康問題,讓個案能在短暫時間內體重增加及營養指標指數回復正常,空腸造 瘻管灌食滲漏問題也得到改善,並善用護理獨有的特質,藉由主動關懷、同理 心及以病人為中心設身處地理解病人需求,適時提供個案及家屬個別性照護, 並強化個案自我照護能力,執行照護技術時給予正向回饋及情緒支持,並提供 相關照護資訊,透過傾聽技巧和家屬的陪伴過程協助個案渡過住院治療期,期 望藉此臨床照護經驗,能提供往後照顧類似個案之參考方向。 |
| 英文摘要 | This paper aims to share the nursing experience of providing nutritional and postoperative jejunostomy care for a patient with esophageal cancer. Nursing care period was from July 26, to August 23, 2022. We utilized Gordon's 11 Functional Health Assessment to evaluate the patient's condition through observation, direct care, physical assessment, and interaction. The assessment revealed four main health issues: "Imbalanced Nutrition: Less than Body Requirements," "Pain," "Impaired Skin Integrity," and "Anxiety." Throughout the care process, significant physiological and psychological desires were identified, including severe malnutrition, postoperative jejunostomy tube feed leakage, postoperative pain, and anxiety about further surgery. Through interdisciplinary teamwork involving physicians, dietitians, and stoma care nurses in order to help improve individual health matters. This results to patient’s weight gain and normalization of nutritional indices within a short period. Improvement was also observed in jejunostomy tube feed leakage, leveraging the unique qualities of nursing, such as proactive care and empathy, a patientcentered approach was adopted to understand the patient's requirements. Personalized care was provided, and efforts were made to strengthen the patient's self-care abilities. Positive feedback, emotional support, and relevant care information were provided during care procedures. Assistance was provided to the patient during the hospitalization period by applying listening skills with the presence of family members. It is hoped that this clinical nursing experience can provide a reference for the future care of similar cases. |
