| 中文摘要 | 本文運用全人照護模式照護一位扁桃腺癌脫離呼吸器之護理經驗,照顧期間自2022年12月17日至2023年1月19日,筆者透過直接照護、會談、觀察及病歷檢閱等方式,以生理、心理、社會、靈性等四大層面評估,確立了呼吸機戒斷反應功能失常、無望感、照顧者角色緊張等健康問題。由於癌細胞壓迫氣管而引起呼吸困難,需放置氣切內管並使用呼吸器維持呼吸功能,協同醫療團隊與病人及其配偶共同制定呼吸器訓練及肺部復健計畫,並在執行計畫過程提供音樂及精油穴位按摩以改善呼吸困難感受。當病人出現擔心疾病治療癒後及缺乏對疾病照護的資訊的無望感,家屬也面臨著照護上的困難,透過召開家庭會議,共同討論目前疾病進展狀況、後續接受呼吸訓練的成效及出院準備服務事項。護理過程中,秉持以全人照護的核心本質,透過用心、用腦及用手的照護方式,與病人及家屬建立友好且信任的護病關係,提供全方位的照護,以確保病人身體舒適,家屬的心也安適。期望透過分享此照護經驗,為臨床護理人員提供相關病人護理之參考。 |
| 英文摘要 | This article is about nursing experience of applying the holistic care on a tonsillar cancer patient for weaning of ventilation. The caring period was from December 17, 2022to January 19, 2023. The author conducted a holistic assessment through direct nursing, conversation, observation, reviewing medical records. By applying four major levels of assessment: physical, psychological, social, and spiritual, to identify health problems such as dysfunction of the respirator withdrawal response, a sense of hopelessness, and strained caregiver roles. Due to tumor compression causing respiratory difficulties and the subsequent need for ventilator support, the patient underwent insertion of a tracheostomy tube to maintain breathing function. The medical team collaborated with the patient and his spouse to develop ventilator weaning and pulmonary rehabilitation training. During the implementation of these plans, music, aromatherapy and acupressure therapy were provided to alleviate respiratory distress. When patients shown hopelessness after the treatment and lack of knowledge regarding disease care, family members will also face care difficulties. Family meetings were held to discuss the updated disease progression, the effectiveness of subsequent respiratory training, and preparations for discharge. Transitional care resources were identified, arrangements were made to ensure the patient receives continuous and high-quality healthcare after leaving the hospital. The caring process was endorsed as the core value of nursing, emphasized by integrating the brain, heart, and hands to foster a supportive environment. A trusting nurse-patient relationship was established with the aim of providing comprehensive care, addressing both the patient's physical comfort and peace of mind for the family. Hopefully by sharing this care experience, it can provide relevant patient care reference for clinical nurses. |
