| 中文摘要 | 本文探討一位30歲年輕女性個案在被診斷為肝癌末期,癌末除了造成病人生理不適,面對無法控制的生命流逝更嚴重衝擊病人與家庭的身心健康及生活品質。護理期間為2022年10月07日至2022年10月25日,筆者藉由觀察、會談、身體評估等方式收集資料,運用「Gordon十一項功能性健康模式」做整體性評估,確立個案健康問題有疼痛、死亡焦慮及家屬預期性哀傷之健康問題。在護理過程中,筆者與個案及家屬建立良好的護病關係並與醫療團隊合作,主動關懷、傾聽與陪伴個案及家屬,運用藥物與非藥物治療等措施給予個別性照護,緩解疾病引起的疼痛不適,經跨團隊合作引導個案及家屬坦然面對癌症,說出彼此內心感受,帶領家屬進行生命回顧,幫助個案面對死亡的恐懼,接受癌病末期的事實,體會生命價值,進而勇於面對生命的盡頭,漸走出哀傷情緒。藉此照護經驗分享提供臨床護理人員做為照護此類個案之參考。 |
| 英文摘要 | This article discusses the case of a 30-year-old young female who was diagnosed with terminal liver cancer. Apart from physical discomfort for the patient, the terminal cancer significantly impacts physical and mental health as well as the quality of life for both the patient and her family due to the uncontrollable passage of life. The nursing care period was from October 7 to 25 in 2022. The author collected data through observations, interviews, physical assessments, and applied Gordon's 11 Functional Health Assessment for a comprehensive assessment. This assessment identified the patient's health issues include pain, death anxiety, and anticipatory grief in family members. During the nursing process, the author established a strong nurse-patient relationship with the patient and her family, collaborating with the medical team for proactive care, listening, and companionship to the patient and their family. Individualized care was provided through both pharmacological and non-pharmacological measures to alleviate the pain caused by the illness. Through interdisciplinary collaboration, the patient and their family were guided to face cancer calmly, express their inner feelings, leading them to conduct life review, and confront the fear of death. They hence accepted the reality of terminal cancer, appreciate the value of life, have the courage to face the end of life, and gradually moving beyond grief. This caregiving experience sharing serves as a reference for clinical nursing professionals when caring for similar cases. |
