| 中文摘要 | 本文青少年因機車車禍,致左上肢完全創傷性截肢,引起身、心衝擊,對青少年生命及發展是一大危機。急診照護期間2022/7/29 07:20至09:25,運用整理性護理評估,發現病人因創傷性截肢後有體液容積缺失、急性疼痛及決策衝突等健康問題,出血致體液容積缺失,行止血及輸液治療,穩定生命徵象;使用疼痛量表評估疼痛,利用非藥物及止痛藥物緩解疼痛,除生理上照護,建立良好護病關係,照會各專科跨領域口語化病情解釋,加上運用多媒體方式,使病人更加瞭解病情,使決策達到共識,本文建議將心理團隊及早介入,加入創傷團隊,以達到身心靈平衡。藉此經驗分享,提升截肢外傷護理品質,提供護理人員臨床應用之參考。 |
| 英文摘要 | This article discusses the case of a teenager who experienced a motorcycle accident resulting in a complete traumatic amputation of the left upper limb, causing physical and psychological impacts and posing a significant crisis towards his life and development. During the emergency care period from 07:20 to 09:25 on July 29, 2022, a comprehensive nursing assessment was conducted. It was found that the patient faced health care issues such as fluid volume deficit, acute pain, and decision-making conflicts after the traumatic amputation. The loss of body fluid volume due to bleeding led the patient to undergo emergency care. Hemostasis and infusion treatment were applied to stabilize vital signs. Pain assessment was conducted by using a pain scale, non-pharmacological and analgesic medications were utilized to alleviate pain. In addition to physiological care, a positive nurse-patient relationship was established. The interdisciplinary team communicated verbally about the patient's condition, using multimedia to enhance the patient's understanding of the situation and reach a consensus when making the decision. This article recommends early intervention by the psychological team, inclusion of the trauma team, hence promotes holistic well-being to achieve physical, mental and spiritual balance. Through sharing this experience, the goal is to enhance the quality of care for amputation trauma and provide nursing staff with a reference for clinical application. |
