| 中文摘要 | 隨著人口老化和相應醫療需求增加,腰椎手術日益普遍成為常見的治療方式。術後疼痛是病人主要問題之一,故手術後疼痛管理成為關注焦點。術後加速康復(ERAS)為整合性照護流程,旨在優化手術過程,改善術後的恢復。本文透過系統性文獻回顧,納入22篇研究,共4,586病人。大多數研究顯示ERAS對腰椎手術後疼痛緩解效果顯著,對於搭配2種以上的非鴉片類止痛藥物,能有效降低鴉片類止痛藥物的使用量。然而,關於手術前、中及後期實施的多模式止痛管理存在分歧,鴉片類及非鴉片類止痛藥的種類及劑量也存在差異性。此外,術後疼痛程度的評估工具各有不同,使研究結果難以比較。建議未來研究應採用實驗性設計提升品質,並進一步探索最佳應用方式及成效。 |
| 英文摘要 | As the population ages and the corresponding medical requirements increases, lumbar spine surgery has become increasingly common as a treatment modality. Postoperative pain is one of the major concerns for patients; thus, postoperative pain management has garnered significant attention. Enhanced Recovery After Surgery (ERAS) is an integrative care process aimed at optimizing the surgical process and improving postoperative recovery. This article conducted a systematic review, including 22 studies with a total of 4,586 patients. Most studies indicated the significant effectiveness of ERAS in reducing pain after lumbar spine surgery. When combined with more than two kinds of non-opioid analgesics, it can effectively reduce opioid consumption. However, consensus has yet reached the analgesic interventions implementation before, during, and after surgery, and there are discrepancies in the assessment tools for postoperative pain intensity, making comparison of study results more challenging. Furthermore, divergent approaches to implementing multimodal analgesia after surgery hinder comparability. It is recommended that future research adopt experimental designs to enhance quality and further investigate to determine the optimal application methods and their efficacy. |
