| 中文摘要 | 本文探討一位60歲男性,此次因肝癌末期面臨身體上的症狀不適,衍生身、心、靈層面等問題入住加護病房之照護經驗。筆者於2021年9月28日至10月9日照護期間,藉由觀察、會談、身體評估及查閱病歷收集資料,運用Gordon十一項健康功能評估,確立個案有「低效性呼吸型態」、「疼痛」、「靈性困擾」之健康問題。照護過程以主動關懷、耐心傾聽並同理個案身心靈需求,透過跨團隊合作儘早使個案脫離呼吸器,以自製深呼吸訓練器提升個案自咳能力改善呼吸型態;運用放鬆及冥想療法、轉移注意力,以提升身體舒適度、緩解疼痛不適;結合安寧共照團隊從「心」再出發、以「愛」做連結,讓個案在生命歷程最終回能了無遺憾,圓滿人生最後一哩路。建議後續可藉由各專家意見整合,針對相關個案行案例討論,提供急重症醫療團隊在面對臨終患者時,應如何與安寧團隊結合,達成高品質的生命末期照護。期許藉由此照護經驗,提供日後照護相關個案之參考。 |
| 英文摘要 | This article explores the nursing experience of a 60-year-old man who was admitted to the intensive care unit with physical symptoms and derived physical, mental, and spiritual problems associated with terminal liver cancer. During the care period from September 28 to October 9, 2021, the author collected data through observation, interview, physical assessment and medical record review, by applying the Gordon's 11 Functional Health Assessment, hence establish that the patient had the health problems of "inefficient breathing pattern", "pain" and "spiritual distress". The process of care was based on active care, listening patiently and empathizing with the physical and spiritual needs of the patient. Through interdisciplinary-team collaboration, the patient was weaned from the respirator as early as possible, and a handmade deep breathing trainer was used to enhance the self-coughing ability and breathing pattern. Relaxation and meditation were adopted to divert the patient's attention to enhance physical comfort as well as relieve pain and discomfort. In collaboration with the hospice team, starting from the "heart" and connecting with "love", so that the patient can complete the last mile of life without regrets towards the end of his life journey. It is recommended that the opinions from various experts can be integrated in the future and case discussions can be conducted on similar cases to provide how the emergency medical team can combine with the hospice team to achieve high-quality end-of-life care when facing terminal patients. It is hoped that this nursing experience can be used as a reference for the care of relevant cases in the future. |
