| 中文摘要 | 本文敘述一位因初次罹患腦中風合併右側肢體偏癱,造成言詞溝通障礙甚至生活失能之護理經驗。個案除了面對極大的自我身心衝擊外,更因為疾病促使整個家庭功能失衡,造成主要照顧者照護上的壓力與負荷。照護期間為2021年3月10日至30日,藉由生理、心理、社會和靈性等層面進行評估,透過觀察及身體評估、會談等方式資料收集與分析,確立個案健康問題為有身體活動功能障礙、無力感、照顧者角色緊張。筆者透過復原力模式之運用,並結合跨領域團隊合作,擬定復健訓練與治療計劃,使其恢復肢體力量及語言能力,改善中風失能者之功能障礙,強化照顧者照護能力與信心。建議將來針對初次腦中風患者,運用復原力於臨床照護上,來提升個案自我效能以因應疾病適應能力,引導個案接納疾病帶來的痛苦與衝擊,有助於病患面對壓力的調適與處理。期盼此照護經驗之分享,日後能提供臨床健康照護者應用於促進中風病人復原力之參考和提升照護品質。 |
| 英文摘要 | This article discusses the experience of caring for a patient who suffered from a stroke for the first time and had hemiplegia of the right limb resulting in communication difficulties, ultimately leading to a loss of independence. In addition to the significant impact on the patient, the disease also caused imbalances in the family function, leading to stress and burden on the primary caregiver. The caring period started from March 10th to the 30th, 2021, assessment was carried out at the physiological, psychological, social and spiritual levels, through observation, physical assessment, interviews and other methods, data collection and analysis was carried out to establish that the patient encountered health issues such as physical activity dysfunction, feeling weak, and caregiver role strain. By utilizing the resilience model and collaborating with a multi-disciplinary team, the author developed rehabilitation training and treatment plans to restore limb strength and language ability, alleviate some functional disability caused by stroke, also strengthen the caregiver's ability and confidence in providing care. It is recommended that resilience to be used in clinical care for initial onset stroke patients in the future to improve the patient's self-efficacy when adapting to the disease, guide the patient to accept the pain and impact caused by the disease, hence help the patient adjust to handle stress. By sharing this care experience, we hope to provide a reference for clinical healthcare providers to improve nursing care quality for stroke patients. |
