| 中文摘要 | 目前外籍家庭看護工(簡稱外看)已成為我國居家高齡照顧的主要人力,但較缺乏針對失智症家庭照顧者雇用外看之經驗的質性研究。本研究以質性研究方法深入了解失智症家庭照顧者雇用外看照顧失智家人之原因、優點與隱憂。本研究以深度訪談法訪談四位失智症家庭照顧者,並就訪談資料進行主題分析。研究發現家庭照顧者綜合評估個人情況與現有資源後,雇用外看作為替代照顧人力方案。優點包括:外看協助減輕照顧負擔,使家庭照顧者能調適個人情緒,並能部分從照顧責任中解放出來並兼顧個人生活。缺點則為:失智家人排斥外人照顧、因政策與大環境變化造成雇用外看的困難與不確定性、外看工作不盡責及缺乏照顧失智症者的知識與技巧。值得注意的是,家庭照顧者增進對失智症的了解後,意識到善用政府長期照顧資源來提升外看照顧識能的重要性。本研究建議政府積極擴充給家庭照顧者及外看的失智症照顧課程,提升兩者的照顧識能,促進失智症者與照顧者良好生活品質。 |
| 英文摘要 | Live-in migrant care workers have become essential for home-based eldercare in Taiwan. However, qualitative studies focusing on the experiences of family caregivers relating to employing migrant workers to care for their relatives with dementia are few. The qualitative study explored why families employed migrant workers to provide home-based dementia care and its strengths and weaknesses. In-depth interviews were conducted with four family caregivers who employed live-in migrant care workers. The interviews were transcribed and analyzed by using thematic analysis. The data showed that family caregivers hired migrant workers to provide home-based care for their relatives with dementia after making appraisals of their personal conditions and available resources. Live-in migrant care workers served as surrogate caregivers, allowing them to alleviate daily care burdens, adjust their emotions, and maintain their personal lives. However, family caregivers also encountered several problems, including their dementia-affected relatives' refusal of care provided by migrant caregivers, the difficulties of hiring a migrant caregiver due to the changing, more expansive environments, and the encounter of a migrant caregiver who was irresponsible or lacked dementia-related knowledge and care skills. As family caregivers' knowledge about dementia improved, they became aware of the importance of using public long-term care services to improve their migrant employees' dementia-related knowledge and care skills. The study suggests that more dementia-specific courses should be provided to both family caregivers and live-in migrant care workers to improve their care knowledge and capabilities, which matters to the quality of life of both the caregivers and the care recipients. |
