| 中文摘要 | 背景:核子醫學在近20年發展迅速與需求急速增加,過往國內研究均以全國性資料分析,然而單一醫療機構更能地區反映第一線病患需求,故本研究乃針對臺灣南部某區域醫院進行19年核子醫學科檢查量趨勢分析,以瞭解當地患者對於核醫需求。方法:採用資料回溯方式蒐集臺灣南部某區域醫院19年之核子醫學科檢查相關資料。本研究採用SAS® OnDemand for Academics進行資料處理與分析,以平均數、標準差等統計方法描述每年核醫總檢查量及各項檢查之檢查量,最後再以趨勢分析檢視「2004至2022年間每年總檢查量」及常見核醫檢查之2004至2022各年度改變趨勢。結果:本研究結果顯示,研究機構2004至2022年核子醫學科平均每年總檢查量為9,081.16 ± 1,578.91筆。在單一檢查項目部分,前三名依序為骨質密度檢查檢查量77,367筆、全身骨骼掃描共45,431筆、心肌灌注掃描共27,283筆。趨勢分析結果顯示,總檢查量隨時間並無顯著變化趨勢,而心肌灌注掃描、全身正子造影檢查(whole body FDG PET/CT)、鎵-67發炎、全身骨骼掃描和淋巴閃爍掃描檢查等五種檢查之檢查量則會隨時間有顯著變化趨勢,而全身骨骼掃描、全身正子造影檢查和淋巴閃爍掃描呈現上升趨勢,其餘兩種檢查項目卻呈現下降趨勢。結論:每年總檢查量並不會隨時間有顯著變化趨勢,但全身骨骼掃描、正子造影檢查和淋巴閃爍掃描檢查等三種檢查項目均會隨時間有顯著呈現上升趨勢。推測每年總檢查量並不會隨時間明顯變化的原因可能為,研究期間內進行儀器設備更新及歷經新冠肺炎期間,降低病人對於核醫利用情形。 |
| 英文摘要 | Background: Nuclear medicine has undergone significant advancements over the past two decades, accompanied by a corresponding increase in clinical demand. While previous studies have primarily relied on national datasets, data from individual medical institutions may offer more precise insights into frontline patient needs. This retrospective analysis examines nuclear medicine examinations at a regional hospital in southern Taiwan over a 19-year period (2004-2022) to assess evolving patient demand and inform strategic planning for nuclear medicine services. Methods: Data processing and analysis were performed using SAS® OnDemand for Academics, a comprehensive tool for statistical and analytical tasks. Descriptive statistics, such as mean and standard deviation, were used to summarize annual total examinations and individual examination types. Trend analyses were performed to evaluate changes in total examination volumes and the volumes of common nuclear medicine examinations over the period of 2004 to 2022. Results: The average annual total examination volume in the nuclear medicine department of the study institution from 2004 to 2022 was 9,081.16 ± 1,578.91. The three most frequently performed examinations were bone densitometry (77,367 exams), whole-body bone scans (45,431 exams), and myocardial perfusion scans (27,283 exams). Trend analysis revealed no significant changes in the total examination volume over time. However, among individual examination types, five showed significant trends: myocardial perfusion scans, whole body positron emission tomography/computed tomography (FDG PET/CT), Gallium-67 scans for inflammation, whole-body bone scans, and lymphoscintigraphy. Whole-body bone scans, FDG PET/CT, and lymphoscintigraphy exhibited increasing trends, while myocardial perfusion scans and Gallium-67 scans demonstrated declining trends across the study period. Conclusions: The total annual examination volume showed no significant temporal trend. Despite factors such as equipment upgrades and the impact of the COVID-19 pandemic potentially reducing patient utilization of nuclear medicine services; whole-body bone scans, FDG PET/CT scans, and lymphoscintigraphy scans demonstrated a significant increase over time. As a result, the annual total examination volume has remained stable. |
