| 中文摘要 | 本文探討個案為一位因腸阻塞行腸沾黏剝離術術後又因感染 COVID-19 進行隔離 之照護經驗。照護期間自 2022/10/02 至 2022/10/13 經由觀察、傾聽、身體評估、會談 及溝通法等技巧收集資料及 Gordon 十一項功能性健康評估做整體性評估,確立個案有 急性疼痛、焦慮及知識缺失等健康問題。針對上述問題,鼓勵表達疼痛感受及同理接納, 適時調整及提供止痛藥物處置,運用舒適臥位、束腹帶使用,並透過精油、放鬆技巧轉 移注意力,使疼痛獲得控制。建立良好的治療性關係,鼓勵說出想法與感受,緩解焦慮 情緒。針對疾病之認知程度給予適切防疫相關衛教工具,提升個案對於疾病之了解。建 議應設置個案管理師進行醫病關係連結機制,透過「Line @」之社群軟體群組及提供「知 識通」,透過遠距資訊推播及遠距醫療諮詢,增進醫病關係、減輕病人及家屬面對疾病 的無助感,提升防疫觀念及照護知識。 |
| 英文摘要 | This article discusses the care experience of a patient who underwent intestinal adhesion dissection due to intestinal obstruction and was isolated due to COVID-19. During the nursing period, from 2022/10/02 to 2022/10/13, through observation, listening, physical assessment, talking and communication techniques and other skills to collect data and Gordon's eleven functional health assessments for overall assessment, it was established that the case had health problems such as acute pain, anxiety and lack of knowledge. In view of the above problems, encourage the expression of pain feelings and empathetic acceptance, adjust and provide painkiller treatment in a timely manner, use comfortable lying position, abdominal belt, and divert attention through essential oils and relaxation techniques to control pain. Build a good therapeutic relationship that encourages speaking thoughts and feelings and relieves anxiety. Provide appropriate anti-epidemic related health and education tools according to the degree of disease awareness to enhance the understanding of the disease in individual cases. It is suggested that case managers should be set up to link up the relationship between doctors and patients, and through the "Line @" community software group and the provision of "Knowledge Communication", through remote information dissemination and telemedical consultation, to enhance the doctor-patient relationship, alleviate the helplessness of patients and their families in the face of illness, and enhance the concept of epidemic prevention and care knowledge. |
