| 中文摘要 | 本文敘述一位 22 歲就學中的年輕女運動員,因感染性心內膜炎入加護病房治療之 護理經驗,護理期間為 2021 年 4 月 9 日至 4 月 26 日,筆者運用 Gordon 十一項功能性 健康型態架構以直接照護、觀察、身體評估、查閱病歷、傾聽及會談方式收集資料,發 現個案有氣體交換障礙、現存性感染及焦慮等健康問題。住院期間全球新冠肺炎疫情蔓 延,探病時間受到限制,增加個案不安及不確定感,除了透過陪伴、傾聽、主動關懷外, 並利用通訊軟體使用與個案和家屬建立良好溝通機制及信任的護病關係,建立個案對於 疾病認識,增加信心並協助擬定後續治療計畫,陪伴個案渡過突如其來的疾病與全球疫 情產生的心理衝擊之過程。讓我們了解到未來在照護重症病人時,除了監測生理數據及 照護外,更要適切給予心理關切及早提供適當心理支持,制定更符合個別性、多元及全 方位之照護計畫。 |
| 英文摘要 | This article describes the nursing experience of a 22-year-old female student athlete who was admitted to the intensive care unit for the treatment of infective endocarditis. The nursing period was from April 9th to April 26th, 2021. The Gordon's Functional Health Patterns framework was utilized to collect data through direct care, observation, physical assessment, medical record review, listening, and conversations. It was found that the patient exhibited health problems such as impaired gas exchange, existing infection, and anxiety. During the hospitalization, hospital visiting time was restricted due to global spread of the COVID-19 pandemic patient's restlessness and uncertainties were increased. Therefore, the nurse used a communication software to establish an effecting communication channel and a trusting nurse-patient relationship with the patient and her family, apart from providing companionship, listening, proactive care, and support. Finally, the patient-understanding of her illness was facilitated, the confidence of her was booted, and formulating a comprehensive treatment plan was aided. The nursing care team accompanied the patient through the sudden illness and the psychological impact caused by the global pandemic. This experience highlights the importance of not only monitoring physiological data and providing care, but also offering appropriate psychological attention and timely psychological support when caring for critically ill patients. It underscores the need for tailored, diverse, and comprehensive care plans to address individual needs. |
