| 中文摘要 | 癌症,稱為當代的「萬病之王」(《The Emperor of All Maladies》),而腦癌尤其惡性而危險,不僅病因不明,預後差,且存活期短,目前西醫的治療雖有手術切除、放射治療、化學治療及標靶治療,但無法避免腦癌的復發,更缺乏預防與避免再復發的方法。本文在中西醫結合的治療基礎,探索腦癌的全人醫療,提出中醫的太極全人觀及「靈性五我」全人觀作為臨床治療的實踐途徑。本文分析了七個高度惡性的腦癌案例,2例Anaplastic oligodendroglioma(AO ,WHO 3級)及2例Anaplastic Astrocytoma(AA, WHO 3級),以及3例膠母細胞瘤(GBM, WHO 4級),所有病患皆接受過西醫治療,並有不同程度的副作用及腦損傷或多次復發。在加入或僅保留中醫治療後,根據醫學影像(MRI、CT)及病理報告,結果顯示,7例中5例病情穩定無復發,PFS(Progression Free Survival,無惡化存活期)為42~14個月,並持續延長;1例AA在終止中醫治療後一年復發,經重新接受中醫治療後PFS已達10個月未再復發;1例GBM病患因熬夜工作,於終止中醫治療後6個月復發。本文提出「三治三陣十三法」治療策略,包含三個治則:一、扶正祛邪,病傷同治;二、位性形勢,辨證論治;三、靈性關懷,全人醫治;十三法「清熱、活血、利濕、化痰;和解、通降、攻毒、補養、瀉下;滋陰、重鎮、開竅、散結」法,則根據不同病程階段,選擇相應的治療方法。此策略作為中西醫結合治療的實證基礎,具有臨床研究價值。 |
| 英文摘要 | Cancer, known as the "Emperor of All Maladies," is especially malignant and dangerous when it comes to brain cancer. The causes of brain cancer remain unclear, its prognosis is poor, and survival rates are low. Although Western medicine offers treatments such as surgical resection, radiation therapy, chemotherapy, and targeted therapy, these methods cannot entirely prevent recurrence and lack preventive measures for avoiding future relapses. This article explores holistic treatment approaches for brain cancer by integrating traditional Chinese and Western medicine. It proposes the use of traditional Chinese medicine's concept of holistic care through the "Taiji Holistic Perspective" and the "Spiritual Five-I" framework as practical approaches in clinical treatment. Seven cases of highly malignant brain cancer, including 2 cases of Anaplastic Oligodendroglioma (AO, WHO Grade 3), 2 cases of Anaplastic Astrocytoma (AA, WHO Grade 3), and 3 cases of Glioblastoma Multiforme (GBM, WHO Grade 4). All patients have received Western medical treatments and experienced varying degrees of side effects, brain damage, or multiple recurrences. After adding or retaining TCM treatments, and based on medical imaging (MRI, CT) and pathological reports from a medical center, the results showed that of the seven cases, five were stable without recurrence, with a progression-free survival (PFS) ranging from 42 to 14 months, and continued to extend. One case of AA relapsed one year after stopping TCM treatment, but after resuming TCM treatment, the PFS reached 10 months without further relapse. However, one case of GBM, a patient who worked night shifts, relapsed six months after discontinuing TCM treatment. The article also discusses the "Three Treatment Principles, Three Strategies and Thirteen Methods": 1. Three Treatment principles: o First, reinforce the healthy qi and eliminate the pathogenic factors, treating both the disease and the damage caused by it. o Second, pattern identification as the basis, for treatment the position, nature, form, and dynamic of disease. o Third, spiritual care and holistic treatment. 2. Three Strategies and Thirteen Methods: o First, clearing heat-toxin, Activate blood and resolve stasis,draining dampness and water retention,transform phlegm and dissipate nodules to treat conditions like heat toxicity, blood stasis, dampness, and phlegm retention. o Second, harmonizing exterior and interior, downflow-freeing, detoxifying, supplementing, purgation and harshly expelling. o Third, nourishing yin and tonify yang, tranquilize by heavy settling, orifice-opening and resuscitation , and dissipate binds and resolve masses & hardness. These methods are integrated to propose a treatment pathway combining Chinese and Western medicine, forming the foundation for evidence-based holistic care in clinical practice. |
