| 中文摘要 | 研究目的:照顧者與失智者之間關係常因病程惡化及失智者精神行為問題產 生負面影響,國外學者建議將音樂活動納入居家照顧,以增加失智者與照顧者之正 向互動經驗;本研究欲探討音樂介入方案對照顧者與失智者關係之影響,以及了解 照顧者對音樂介入方案之反應。研究方法:質量並重的平行混合研究方式,以12周 的音樂介入培訓方案作為介入措施,參與者在課程開始前、後一周進行量表填寫及 訪談,量表以無母數統計之Wilcoxon符號等級檢定進行差異考驗,訪談內容包含整 體方案參與經驗及回饋,根據訪談錄音撰寫逐字稿,作為質性分析資料。研究結果 :僅正向照顧經驗有顯著差異(p<0.05),相互關係的Cohen’s d=0.642呈現中等以 上的效果量,質性分析結果顯示參與者出現三項共同主題,包含改善相互關係、照 顧酬賞、以及影響參與者本身。研究結論:讓照顧者在居家環境中與失智者一同進 行音樂活動,對於兩人之間的照顧關係有正向幫助,培訓課程、活動手冊與樂器、 家訪並給予照顧者個別化建議等,對於照顧者來說皆能提供助益,故建議未來可運 用音樂介入培訓方案幫助失智症患者之照顧者。 |
| 英文摘要 | Purpose: The number of dementia caregivers has been rapidly increasing, and the relationship between caregivers and dementia patients is often negatively affected by disease progression and the behavioral issues of dementia patients. While past research has explored the use of music therapy to assist dementia patients, recent international reports have suggested incorporating music activities into home care to increase positive interactions between dementia patients and caregivers and to improve the caregiving relationship. This study aims to investigate the impact of a music intervention program on the relationship between dementia caregivers and dementia patients, as well as to understand the feedback of dementia caregivers to the music intervention program. Method: This is a pre-experimental study with a single-group pretest-posttest design, employing a mixed-methods approach with an equal emphasis on both quantitative and qualitative data collection. A 12-week music intervention training program was implemented as the intervention. Participants completed questionnaires and interviews one week before and after the program. Non-parametric Wilcoxon signed-rank tests were used to assess differences in questionnaire responses, and interview transcripts were analyzed qualitatively. Results: Significant differences were observed only in the Positive Aspects of Care scale (p<0.05). Although there were no significant differences in the interpersonal relationship aspect, Cohen’s d= .642 indicated a medium to large effect size. Qualitative analysis revealed three common themes among participants, including improving interpersonal relationships, caregiving rewards, and personal impacts. Conclusion: Allowing caregivers and dementia patients to engage in music activities within a home environment has a positive impact on the caregiving relationship. The elements of this program, including the training curriculum, providing handbooks and musical instruments, conducting home visits, and offering personalized recommendations to caregivers, all proved beneficial. Therefore, it is recommended that music intervention training programs be utilized to assist caregivers of dementia patients in the future. |
