| 英文摘要 | Background: This case report examines the effect of standardized weighted low-resolution electromagnetic tomography Z-score neurofeedback (swLZNFB) on symptoms of depression, anxiety, sleep disturbance, and electroencephalography (EEG) parameters in a patient comorbid with major depressive disorder (MDD) and anxiety symptoms. Method: The patient is a 27-years-old single female who experienced emotional distress due to relationship issues and has been diagnosed with MDD since her senior year of high school. She reported persistent depressive moods, feelings of worthlessness, severe insomnia, and anxiety. Despite taking sleep medication, she continued to have difficulty falling asleep. Her psychiatrist referred her for 10 sessions of swLZNFB training, with each session lasting 1.5 hours, conducted twice a week over a 5-week period. EEG data was converted to current source density and standardized to Z-scores by comparing it with normative database from NeuroGuide software. The swLZNFB training protocol targeted emotional brain regions using various frequency bands and EEG parameters (power, coherence, lagged coherence, and phase) based on a symptom checklist for depression and anxiety. Depression and anxiety symptoms, pre-sleep arousal, sleep quality, and EEG parameters were assessed before and after the swLZNFB training. Results: This study applied the theory of stress, appraisal, and coping for case conceptualization. After completing the swLZNFB training, the client showed the following improvements: (1) A decreased in depressive and anxiety symptoms, decreased pre-sleep arousal, and improved sleep quality. (2) A decreased in Z-scores of beta bands in the frontal, temporal, and parietal lobes, as well as in high-beta Z-scores across the entire brain. (3) A decreased in the percentage of abnormal EEG values from 42% to 14%. Conclusion: The swLZNFB training led to improvements in depressive and anxiety symptoms, decreased pre-sleep arousal, improved sleep quality, and normalization of brain activity and abnormal EEG parameters in a patient with MDD. This study suggests that swLZNFB may be a valuable treatment option for patients with MDD in future clinical practice. |
