| 中文摘要 | 本文敘述一位43歲男性初次腦中風復健個案之護理經驗。護理期間自2023年11月2日至2023年11月27日,運用Gordon十一項功能性健康型態進行評估,藉由身體評估、會談、觀察及病歷查閱等方式收集資料,確立個案有身體活動障礙、自我照顧能力缺失、無力感之護理問題。照護期間與個案建立良好護病關係,透過主動關懷及傾聽陪伴,鼓勵個案表達心中感受,減少負向情緒,與醫療團隊共同討論擬定復健治療計劃,提供整體性、個別性護理措施,運用專業知識及技能,激發個案復健動機,協助肢體肌力恢復至最大功能,提高日後居家自我照顧的能力而改善生活品質,透過家庭成員及照顧者給予正向支持,增加對於復健治療的配合度及自信,最終使其日後能得到良好的身心調適,回歸正常的生活,正向迎接未來,期望藉此照護經驗分享,提供臨床醫護同仁未來照顧此類個案之參考,有助於達到更完善的護理品質。 |
| 英文摘要 | This article describes the nursing experience of a 43-year-old male undergoing rehabilitation following his first stroke. The nursing period was from November 2, 2023, to November 20, 2023. The Gordon's 11 Functional Health Patterns to data was collected through physical assessments, interviews, observations, and medical record reviews, including impaired physical activity, self-care deficit, and Powerlessness. During the care period, a strong nurse-patient relationship was established, fostering active listening and encouragement for the patient to express his emotions, thus reducing negative feelings. Collaborating with the medical team, a rehabilitation treatment plan was devised, incorporating holistic and individualized nursing interventions. Professional knowledge and skills were applied to motivate the patient towards rehabilitation, aiding in the restoration of limb strength to maximum functionality and enhancing future self-care abilities, thus improving overall quality of life. Positive support from family members and caregivers increased the patient's compliance and confidence in rehabilitation therapy, leading to successful physical and psychological adaptation and a return to normal life, with a positive outlook towards the future. It is hoped that sharing this nursing experience will serve as a reference for clinical healthcare professionals in caring for similar cases, contributing to enhanced nursing quality. |
