| 中文摘要 | 急性心肌梗塞死亡率逐年上升,統計25~44歲因心血管疾病死亡人數由2018年的9.2%上升至2020年的9.9%,著實不容忽視。本文敘述一位於遊樂場發生急性心肌梗塞的青壯年,到院前死亡之急診經驗,護理期間為2020年12月22日至12月23日。照護過程對於青壯年急救後能恢復心跳及心理感受而引發收案動機。筆者運用Gordon十一項健康功能型態評估,藉由直接照護、身體評估、溝通及會談,收集個案生、心理、社會及靈性資料,確立主要健康問題有組織灌流改變、疼痛、焦慮。經醫護團隊介入有效之急救措施,使個案疾病獲得改善;針對心理層面,鼓勵個案使用筆談表達內心的焦慮,並於執行治療及護理前,詳細解說過程及意義、運用有效減輕焦慮技巧,減緩個案焦慮。然而溝通技巧非一蹴可及,且有效的護病溝通需建立於信任基礎上。期望藉此經驗,提升急診有效護病溝通,提供更好的急診照護品質。 |
| 英文摘要 | The mortality rate of acute myocardial infarction is increasing year by year. According to statistics, the percentage of the number of people aged 25-44 who died from cardiovascular diseases increased from 9.2% in 2018 to 9.9% in 2020, which cannot be ignored. The purpose of this study is to describe the experience of emergency nursing on a young adult who suffered from acute myocardial infarction in an amusement park and died before reaching the hospital. The nursing period was from December 22 to 23, 2020. The motivations for taking this patient as the subject were the recovery of the young adult's heartbeat after first aid and his psychological feelings regarding the nursing process. Gordon 11 Function Health Patterns was employed for assessment. The researcher collected the physical, psychological, social, and spiritual data of the subject, and identified his main health problems as altered tissue perfusion, pain, and anxiety through direct care, physical assessments, observations, communication, and interviews. The disease of the subject was improved through the intervention of effective first-aid measures by the medical and nursing team. At the psychological level, the subject was encouraged to express his anxiety by writing, and the process and significance were explained in detail before the implementation of treatment and nursing. Effective anxiety reduction techniques were used to alleviate the subject's anxiety. However, communication skills are not readily available, and effective nurse-patient communication should be based on trust. It is expected that this nursing experience can be used to improve emergency effective nurse-patient communication to provide emergency care with a better quality. |
