| 中文摘要 | 基於正念的干預(Mindfulness-Based Interventions,以下簡稱MBIs)被作為基本補充治療廣泛應用於腫瘤學和緩和醫療,對癌症患者帶來諸多益處已經被大量研究支持,但MBIs為何可以起作用的前因尚未明確,MBIs是如何影響健康的機制也不夠清楚。在諸多論及正念療效的文獻中,同時提及靈性(spirituality)相關的議題,但二者關係尚未明確。本文通過分別對MBIs和靈性需求的滿足之內涵、做法、特徵和療效的分析,發現MBIs療效和靈性需求滿足之間存在密切的聯繫,並論證靈性需求的滿足作為MBIs得以有效的前因之可能性是存在的。基於這種可能性,提出對緩和醫療中的癌症患者採用MBIs有必要主動增加對靈性需求及其滿足的考慮,並對緩和醫療與健康社會工作實務提出三點建議:透過MBIs與靈性關懷提供身-心-靈關懷、運用MBIs和靈性關懷的相輔相成、發展更整全的方法來回應身-心-靈連續體的需求。本研究以已發表的研究成果為研究資料,因大量的文獻都將MBIs作為一個整體來對待,極少文獻對MBIs中包含的具體方法做分別探討,因此使用MBIs這一概括性的概念來展開論述,未對其採用的具體方法,如瑜伽呼吸、冥想等進行比較或交互影響分析,對於具體哪種方法更能夠回應靈性需求或更見療效尚不清楚,是為本研究局限之處。此外,關於對癌症患者的介入具體方法的最低強度和持續時間的研究缺乏。後續的研究可以在前述方面做進一步細化,以利緩和醫療中干預理論的發展和介入措施的完善。 |
| 英文摘要 | Mindfulness-based Interventions (MBIs) are widely used as essential complementary therapies in oncology and palliative care because of their widely proven benefits to cancer patients. However, little is known about antecedents to MBIs' effectiveness, and the mechanisms by which MBIs affect health are not clear enough. Many of literatures on efficacy of mindfulness mention spirituality, but the relationship between them has not been revealed. By analyzing the connotation, practice, characteristics and efficacy of MBIs and spiritual needs satisfaction, this paper finds that there is a close relationship between MBIs efficacy and satisfying spiritual needs, and demonstrates that the possibility of fulfillment of spiritual needs as an antecedent for MBIs to be effective exists. Based on this possibility, it is necessary to proactively increase consideration of spiritual needs and their fulfillment for cancer patients undergoing palliative care when proposing the adoption of MBIs. Three suggestions are offered for palliative care and health social work practices: providing holistic care for the body, mind, and spirit through MBIs and spiritual care, applying the supplementing each other of MBIs and spiritual care, and developing more comprehensive approaches to address the needs of the body, mind, and spirit continuum. This study is based on published research results. Since a large number of literatures treat MBIs as a whole, few literatures discuss the specific methods contained in MBIs. In this paper, the general concept of MBIs is used for discussion, and the specific methods adopted in MBIs, such as yoga breathing and meditation, are not compared or analyzed for interactive impact. Thus, the limitation of this study is that it is not clear which specific approach is more responsive to spiritual needs or more effective. Additionally, research on the minimum intensity and duration of interventions for cancer patients is lacking. Subsequent research can further refine these aspects to facilitate the development of intervention theories and the improvement of intervention measures in palliative care. |
