中文摘要 | 本文描述一位慢性思覺失調症病人因疾病慢性化,導致自我照顧功能嚴重退化,影響生活自理、家庭互動之護理經驗。筆者於2021年5月27日至6月10日間運用五大層面進行整體性護理評估,歸納病人主要有自我照顧能力缺失、照顧者角色緊張之健康問題。筆者以賦權策略為照護基礎,建立信任之護病關係後,透過「自我覺醒、行動、評價」三階段賦予病人高度自主權及參與度,成功激發其改變動機。經由提供專業知識,建構、重整生活自理計畫,養成自我清潔能力並成功改善長期依賴尿布如廁之陋習;於嚴峻疫情下善用電話教導主要照顧者居家照護技巧,降低關係的緊張且增加照顧信心。建議病房應主動、定期召開家屬會談,為病人、家屬與醫療團隊搭起溝通橋樑,賦予充分自主權,使病人正向因應疾病,提升其自我照顧能力以獲得較佳生活品質。 |
英文摘要 | This paper describes the nursing experience concerning a patient with chronic schizophrenia, whose condition led to severe deterioration in self-care capability, affecting her ability to engage in activities of daily living and family interactions. A comprehensive nursing assessment across 5 dimensions was conducted from May 27 to June 10, 2021, and the patient's main health problems as self-care deficit and caregiver role strain were summarized. Utilizing empowerment strategies as the foundation of care, the author established a trusting nurse-patient relationship. Through a three-phase process that involved self-awareness, action, and evaluation, the patient was granted a high degree of autonomy and participation, sparking changes in their behavior. By providing professional knowledge and reconstructing a self-care plan, the author helped the patient developed the ability to perform hygiene tasks on their own and successfully overcame a long-term dependency on diapers for toileting. Amid the severe COVID-19 pandemic, the author also effectively used telephone to provide instructions on home care skills for the primary caregiver, thereby reducing relational tension with the patient and increasing caregiving confidence. According to the aforesaid nursing experience, wards should proactively organize family meetings on a regular basis to establish communication between patients with schizophrenia, their family members, and relevant medical teams. Patients should also be granted full autonomy, which helps them cope with their illness through a positive attitude. These measures can enhance patients' self-care abilities and their quality of life. |
