中文摘要 | 本文描述一位面臨腎臟替代療法抉擇的糖尿病腎病變男性,照護期間為2020年4月15日至2020年7月22日。因個案缺乏透析相關知識、對決策感到不確定而產生抉擇衝突,心理出現焦慮情緒,故引發筆者動機。藉由會談、觀察、電訪、身體評估收集資料,運用生理、心理、靈性及社會層面進行整體性評估,確立健康問題為抉擇衝突、健康尋求行為、營養知識缺失。照護過程中運用醫病共享決策,協助個案依自身想法選擇適合透析模式,並提供後續照護知識及個別性營養計畫增進認知。同時藉由重要他人提供正向支持,成功幫助個案適應並與疾病共存,使個案健康照護最大化。建議未來可定期舉辦醫病共享決策教育訓練,並平行推展至各類疾病的醫療決策情境,將能使更多病人及家屬受惠。 |
英文摘要 | This article reports a man with diabetic kidney disease who faced the choice of renal replacement therapy. The nursing care period was between April 15, 2020 and July 22. According to the deficiency of disease knowledge, this patient could not make the decision whether chose renal replacement therapy, and further had anxiety mood. We arranged face-to-face and telephone interviews, and evaluated his clinical condition through physical, psychological, spiritual and social aspects to diagnose his health problems including conflicts of making decision, health-seeking behaviors, and deficiency of nutrition knowledge. The tool of Sharing Decision-Making was used to assist him in choosing dialysis model based on his thoughts, and we further provided disease knowledge education and individual nutritional management. Adequate support system could successfully help the patient to coexist with the disease. We recommend to organize Sharing Decision-Making education programs, and promote Sharing Decision-Making in various clinical settings to further improve health status of patients and their families. |
