中文摘要 | 隨著末期生命接受善終的觀念越被國人重視,但醫護人員對末期病患家屬經歷代為簽署「不施行心肺復甦術」(DNR)時面臨的困境卻不清楚。本研究採質性研究法,從家屬角度,探討末期病患家屬經歷代為簽署DNR的心路歷程,透過深度訪談,以立意取樣方法招募九個已逝世的末期病患家庭之10位家屬參與研究。資料收集後採質性資料分析法,研究結果呈現病患家屬於獲知病患可能即將離世至簽署DNR後,家屬經歷三個階段的心境:「簽署DNR前的情緒衝擊」、「簽署DNR後心情轉換」以及「簽署後陪病的心理負荷」研究結果有助於護理人員了解家屬在討論簽署DNR至親人離世期間其心理衝擊和需求。建議醫護人員應強化溝通、諮商的專業技能,提供病患與家屬安寧照護相關服務,以滿足家屬之需要。 |
英文摘要 | In Taiwan, people gradually value and accept the importance of singing DNR (Do Not Resuscitate) consent. However, there is a lack to understand the difficulties while families face the event about signing DNR consent for their terminal-stage relatives. In this study, we explored family members' experiences during the period when their relatives were told to sign DNR consent until the death of their relatives. Using a qualitative approach based on purposive sampling, 10 participants representing 9 families participated in in-depth interviews. Qualitative dada analysis method identified three stages of psychological journey: the emotional impact before signing the DNR consent, mood transformation after signing the DNR consent, and the psychological burden during accompanying process after signing. The findings help raise awareness of the process which families with terminal-stage patients sign the DNR consent in Taiwan. Through gaining insights into psychological impacts of terminal-stage patients and their family members, health professionals should deliver a palliative-care consulting service to meet families' needs is suggested. |
