中文摘要 | 目的:一位90歲女性超高齡糖尿病長者,因車禍造成左下肢開放性骨折及大型撕裂髒污傷口,因個案擔憂會截肢有風險,筆者運用實證搜尋最佳傷口照護方法以促進恢復。方法:應用PICO方式搜尋實證文獻,P:開放性骨折、I:負壓傷口治療、C:常規換藥、O:傷口感染率,使用關鍵字搜尋五個資料庫,共獲得27篇,經仔細評讀後納入5篇證據佳之實證文獻,運用2020CASP實證文獻評析工具並統整照護步驟,結果指出負壓傷口治療比常規換藥,更可顯著降低開放性骨折患者的傷口感染率。結果:運用負壓傷口治療運用於此超高齡糖尿病個案之照護,有效改善傷口感染率。結論:負壓傷口治療運用於超高齡糖尿病個案傷口照護,可促進恢復,降低家屬照護負擔。 |
英文摘要 | Objective: A 90-year-old woman with ultra-elderly diabetes mellitus has an open fracture of her left lower extremity and a large laceration and dirty wound caused by a car accident. The case is worried about the risk of amputation. The author uses empirical evidence to search for the best wound care method to promote recovery. Method: Use PICO method to search empirical literature, P: open fracture, I: negative pressure wound therapy, C: conventional dressing change, O: wound infection rate, use keywords to search five databases, and obtain a total of 27 articles. After the review, five empirical literatures with good evidence were included, and the 2020 CASP empirical literature review tools were used to integrate the care steps. The results pointed out that the negative pressure wound therapy can significantly reduce the wound infection rate of patients with open fractures than conventional dressing changes. Result: The application of negative pressure wound therapy in the care of this super-aged diabetic case can effectively improve the wound infection rate. Conclusion: The application of negative pressure wound therapy in wound care of super-aged diabetic cases can promote recovery and reduce the burden of care for family members. |
