中文摘要 | 當顴弓骨折發生時,常常造成患者臉部塌陷,或伴隨開口受限,此時進行手術復位是必要的。本篇個案報告記錄兩位顴骨閉鎖單純性骨折的病人,在局部麻醉下經由口内切線進行了開放性手術復位。術中復位後照X光(下頦頂點方向影像)以確認復位,評估治療效果。術前張口受限的病人術後張口度立即增加了約10公厘。由於在門診局部麻醉下進行手術,免除全身麻醉的風險以及術後移除氣管內管的不適感,手術時間短,術後不需住院,節省了醫療資源。此外術後追蹤時,病人的張口功能和臉部對稱性也有令人满意的結果。 |
英文摘要 | When collapse of patient's malar prominence or limitation of mouth opening are caused by zygomatic fracture, surgical intervention is indicated. In our case report, we introduced two patients zygomaticomaxillary complex (ZMC) fracture and performed outpatient surgery including open reduction via buccal sulcus approach (Keen approach) without internal fixation under local anesthesia. Submentovertex film was checked to confirm achievement proper bone reduction during surgery. The mouth opening increased 10 mm immediately postoperatively in the patient with trismus preoperatively. Due to the operation under local anesthesia, the risks of general anesthesia and the discomfort after removing endotracheal tube are avoided. Our two patients didn't need hospitalization. In general, the outcomes are cost-effective, time-saving, and safe. Our patients all felt satisfied about the outcomes of function and facial symmetry in the postoperative follow-up. |
