| 中文摘要 | 齒源性角化囊腫是源自於早期的外胚層牙板的齒源性良性囊腫。最常發現的位置在下顎後牙區至上升支區域,較長發生在男性。組織學上,齒源性角化囊腫的上皮為上皮角化不全,上皮有六至八層細胞厚度組成,面對其基底膜之底層被極化。齒源性角化囊腫雖被世界衛生組織認為是良性的,但具有局部高度侵襲性。齒源性角化囊腫中的一種上皮正角化亞型,被認為是不同的病變並且不具有侵略性。而上皮角化不全的亞型由於其侵襲性的本質,在切除術或摘除術後具有25-60%的復發率。我們提出一個侵犯至左側下顎骨體及上升支之大型齒源性角化囊腫的病例報告,在囊腫減壓後追蹤一年半有明顯改善。 |
| 英文摘要 | Odontogenic keratocysts (OKCs) (formerly known as keratocystic odontogenic tumor) are benign cystic tumors of dental origin arising from the primitive dental lamina. They commonly occur in the mandible (the posterior body and ascending ramus) and have a slight male predilection. Histologically, OKCs are marked by parakeratinization, a tumor lining six to eight cells thick, and polarization of the basal layer on the basement membrane. OKCs are thought to be benign, but locally aggressive. An orthokeratinized subtype can also occur, but is thought to represent a different lesion and not be aggressive. The parakeratinized subtype has a recurrence rate of 25-60% following cystectomy or enucleation due to its aggressive nature. Here we present a case of a large OKC involving the left ascending ramus of the mandible, as well as its drastic result following decompression. |
