中文摘要 | 背景:顏面骨骨折常需要進行外科復位固定。在台灣,摩托車車禍為導致臉部創傷及顏面骨骨折主要原因。目的及目標:研究及分析顏面骨折模式與摩托車車禍的相關性。材料及方法:回顧性地納入林口長庚紀念醫院急診科接受手術治療的面部骨折患者於2013年9月至2015年11月。收集創傷機制、骨折類型、骨折數(單個或多個)、外傷嚴重度分數(ISS)、新外傷嚴重度分數(NISS)和患者入院時間等參數。根據年齡和損傷機制,通過這些次群體組對患者進行評估。結果:我們分析了503名患者和1018名骨折數,其中年輕男性在樣本中占多數。摩托車事故是主要的傷害機制(73.6%),其次是跌傷(10.9%)。顴骨和下頜骨骨折的發病率最高。與非摩托車組相比,摩托車組年輕患者的顴骨和Le Fort I骨折發生率明顯較高,嚴重程度比例較高,ISS和NISS較高。住院時間無顯著差異。結論:摩托車事故主要是台灣的顏面骨骨折成因。在摩托車事故中,顏面部骨折的年輕患者ISS和NISS顯著升高。此外,摩托車組顴骨和下頜骨骨折的發生率更高。 |
英文摘要 | Background: Motorcycle is the leading mode of transport involved in traffic accidents and subsequent facial bone injuries in Taiwan. Aim and objectives: We examined the pattern of facial bone fractures in Taiwan and their relationship with motorcycle-related accidents. Materials and Methods: From September 2013 to November 2015, patients with facial bone fractures who underwent surgery after being admitted to the emergency department of Chang Gung Memorial Hospital, Linkou, were retrospectively enrolled. The trauma mechanism, fracture type, fracture counts (single or multiple), trauma injury severity score (ISS), new injury severity score (NISS), and admission length of patients were collected. Patients were evaluated by these subgroups based on age and injury mechanism. Results: We analyzed 503 patients and 1018 fracture counts, with young men being predominant in the sample. Motorcycle accident was the leading injury mechanism (73.6%), followed by fall injury (10.9%). The incidence of zygomatic and mandible fractures was the highest. Compared with the nonmotorcycle group, there was a significantly higher incidence of zygomatic and Le Fort I fractures, a higher severity ratio, and higher ISS and NISS in the younger patients in the motorcycle group. Length of admission showed no significant difference. Conclusion: Motorcycle accidents mainly cause facial bone fractures requiring reconstruction in Taiwan. Younger patients with facial bone fractures sustained in motorcycle accidents have significantly higher ISS and NISS. Moreover, the incidence of zygomatic and mandibular fractures was higher in the motorcycle group. |
