中文摘要 | 目的:本研究針對失智症患者居家藥物使用現況,以及老人潛在不適當用藥使用現況進行探討。方法:以某區域教學醫院居家整合照護計畫收案之失智症個案,以問卷模式收集居家藥物使用評估資料,包含基本資料、藥物使用能力、用藥配合度、使用藥品清單。結果:2017年11月至2018年07月間,共收案31人,平均年齡為82.3±8.0歲,男性佔35.5%,個案平均使用6.8±3.5項藥品,51.6%符合多重用藥定義,64.5%個案正在使用至少一種,符合Beers Criteria定義之潛在不適當用藥。用藥配合度測量結果中,48.4%個案表示有時會忘記吃藥。結論:失智症患者的居家照護,需要照顧者與醫療團隊合作,提高藥物使用配合度及預防因藥物使用而造成風險,有助於提升照顧成效。 |
英文摘要 | Objective: This study intended to investigate the current status of home medication use and potential inappropriate medications (PIMs) use among dementia patients. Methods: The dementia cases were collected from the home-based integrated care program of a regional teaching hospital. Questionnaires were used to collect the basic characteristics, medication use ability, medication adherence, and drug use list of the patients. Results: A total of 31 patients were collected from November 2017 to July 2018. The average age of the sample was 82.3 ± 8.0 years with a male ratio of 35.5%. The average medication use was 6.8 ± 3.5 drugs among which 51.6% were consistent with the definition of polypharmacy, and 64.5 % of the patients were using at least one of the PIMs defined by the Beers Criteria. For medication adherence, 48.4% patients claimed that sometimes they forgot to take medicine. Conclusion: Home care for patients with dementia requires the caregiver to work with the medical team. Improvement of medication adherence and prevention of hazards caused by using PIMs help improve the effectiveness of care. |
