中文摘要 | 跨專業團隊之復健醫療照護可以使病人身心功能、情緒、營養與行動力有明顯的進步。中風後的病人因身體功能受損、吞嚥障礙等生理不適,社會和心理問題之多種因素,可能導致營養不良的高風險進而影響生活品質。營養師在PAC的角色,除了完成MNA的評估外,也依病人的營養需求及飲食攝取狀況,與病人及家屬、醫療團隊及供膳人員討論溝通,調整最適合病人的飲食種類。配合吞嚥訓練的進展調整適當的飲食質地,在移除鼻胃管後監測病人飲食狀況,確保其能攝取到足夠的營養及水分,對病人及家屬進行營養衛教,指導正確的飲食知識及行為,避免營養不良或再次中風的風險,以及提供完善的出院準備服務。 |
英文摘要 | Interdisciplinary rehabilitation medical care provided by different teams can make significant improvement in patient physical and mental function, mood, nutrition, and mobility. Post-stroke patients with impaired mobility, physical discomfort such as dysphagia, and multiple factors of social and psychological problems may lead to a high risk of malnutrition and affect the quality of life. In addition to completing the MNA assessment, the roles of a dietitian in post-acute care include adjusting patient diet according to nutritional needs and dietary intake conditions, and communicate with patients and their families, medical teams, and diet suppliers to obtain the best optimized dietary texture for the pa-tients; monitor patient diet conditions after the removal of nasogastric tube to ensure adequate nutrients and water in parallel with the progress of swallowing training; provide nutritional education to patients and their families with the cor-rect dietary knowledge and behavior to avoid the risk of malnutrition or re-stroke, and to provide discharge preparation services. |
