中文摘要 | 過去二十年來,歐美國家的抗精神病藥物的使用在兒童與青少年族群日趨頻繁, 尤其是第二代抗精神病藥物在急躁易怒行為的使用率。這現象普遍受到重視,但亞 洲,乃至於台灣的相關研究文獻及流行病學資料仍付之闕如。是以,本研究採回溯性 資料庫研究,為探討2004年至2013年兒童與青少年抗精神病藥物之處方型態與趨勢, 以某一精神專科醫院門診健保申報檔資料庫分析,針對18歲以下人口為研究對象,並 以 Cochran-Armitage test 進行趨勢雙尾檢定。結果,於2004至2013年間,兒童與青少 年抗精神病藥物之使用由13.63%增加至15.02%(p < 0.001)。其中,第一代抗精神病藥 物的使用由8.05%下降至4.97%,降幅率為38.26%(p < 0.001);第二代抗精神病藥物的 使用由5.84%增加至10.59%,增幅率達81.33%以上 (p < 0.001)。 |
英文摘要 | Over the past two decades, the use of antipsychotics in Europe and the United States has become more frequent in children and adolescents, especially in the use of second generation of antipsychotics for behavioral indication. This phenomenon has generally been taken seriously, but relevant research and epidemiological data in Asia and Taiwan are still scarcity. Therefore, this study adopts a retrospective database of out-patient health insurance filing files of a psychiatric center to investigate the prescription patterns and trends of antipsychotics among children and adolescents from 2004 to 2013. The subjects of the study included were patients under 18 years old. Cochran-Armitage tests were performed for trend analysis. As a result, the use of antipsychotic drugs for children and adolescents increased from 13.63% to 15.02% between 2004 and 2013 (p < 0.001). Among them, the first generation of antipsychotic use decreased from 8.05% to 4.97%, the rate of decrease was 38.26%(p < 0.001); the use of second-generation antipsychotic drugs, by contrast, increased from 5.84% to 10.59%, an increase rate above 81.33%(p < 0.001). |
