中文摘要 | 背景:常規的游離肌肉瓣手術術後監測常常很困難,在文獻中有很多的皮瓣監測方法被提出,這些方法通常會需要額外的花費及支出,然而在鄉下地區,資金及醫療照護人員也相對有限的醫院,外科醫師需要一個簡單的方法來監測游離皮瓣。目的及目標:我們設計一個簡單、可靠、準確及便宜的方法來做游離肌肉瓣的術後監測。材料及方法:我們在取肌肉瓣的時候保留一部份的皮膚作為直接觀察的部分,我們收入2011年4月到2014年4月,在大林慈濟醫院使用帶有部分皮膚的游離肌肉瓣來重建下肢缺損的病人。結果:一共收入20位病患接受21塊肌肉瓣手術,其中有14位男性病人,6位女性病人,平均年紀是61.2歲,沒有完全壞死的案例。結論:使用監測皮瓣已經不是一個新的想法,但是它是一種便宜、簡單與有效的方法來做游離肌肉瓣的監測,不需要購買新醫療設備的額外費用,這種方法很適合資本預算有限的醫院和外科部門。 |
英文摘要 | Background: Conventional methods of post-operative monitoring of free muscle flaps are difficult. There are a myriad of methods of flap monitoring that have been described in literature. However these all involve the acquisition of additional, often costly, medical equipment. In countries where funds and health care personnel are limited, surgeons need an easy method of monitoring the success of a free flap transfer. Aim and Objectives: We design a simple, reliable, effective and cheap method of monitoring the success of a free muscle flap transfer. Materials and Methods: We describe a method of including a skin paddle with the muscle flap to allow direct visualization. This accurately reflects the viability of the muscle flap. All patients who underwent lower limb reconstructions with a free muscle flap using a skin paddle as a monitor flap between April 2011 and April 2014 at Dalin Tzu Chi General Hospital, were included in this study. Results: A total of 21 flaps were performed on 20 patients. There were 14 males and 6 females with a mean age of 61.2 years old. No total flap failure was encountered. Conclusion: The idea of using a monitor flap is not new, but incorporating it as part of a free muscle flap allows for a cheap, simple and effective method of flap monitoring. The additional expense of purchasing new monitor equipment is not required. This method is most suitable for hospitals and surgical facilities where funds are limited. |
