中文摘要 | 背景與目的:對偏癱患者而言擁有良好的平衡能力來預防跌倒,並保持生活自理和獨立行走能力,是影響偏癱患者生活品質的重要因素之一。下肢阻力訓練可以有效的改善肌力及爆發力並可能使平衡能力增進,但關於爆發力與平衡關係的研究較少。本研究旨在探討偏癱患者的肌力和爆發力何者才是增進平衡能力的主要能力,並評估三者的相互關係。方法:偏癱患者共35人次參與,平均年齡為54.3±17.9歲,針對下肢做阻力訓練共有36次,每周進行2次共訓練18周,介入方式以下肢氣壓式阻力訓練機進行,其動作主要有三種:水平下肢推蹬、膝關節伸直和膝關節彎曲,並評估測驗前後的肌力、爆發力和平衡能力包含:伯格氏平衡量表(Berg balance scale, BBS)和平衡測量儀(computerized posturography)。結果:在阻力訓練後的肌力和爆發力皆有明顯進步並達統計上的差異(p< .01;p< .01.),在訓練後測量平衡能力之伯格氏平衡量表的表現上也有進步並達統計上差異(p< .05),但在另一項平衡測量儀的測試中有改變的趨勢但卻未達統計上之差異;將統計上有明顯差異的改變量參數再作相關分析,發現伯格氏平衡量表與下肢肌力有正相關:下肢推蹬(r=.46,p=.005);膝關節伸直(r=.59,p<.001);膝關節彎曲(r=.54,p=.001),和爆發力之下肢推蹬(r=.51,p=.002)、膝關節伸直(r=.44,p=.009)呈正相關,但在膝關節彎曲這一項並未達統計上顯著差異。結論:下肢阻力訓練可以改善偏癱患者的平衡能力,所以建議可將下肢阻力訓練列入偏癱患者的常規訓練之一。 |
英文摘要 | Background and Purpose: Balance ability is one of the important influencing factors of the quality of life of hemiplegic patients. It is to prevent falls and maintain self-care and independent walking ability. Lower extremity resistance training can effectively improve muscle strength, muscle power, and balance. However, studies focused on balance and muscle power are scarce. Therefore, this study was aimed at investigating the effects of resistance training interventions on the balance ability of hemiplegic patients. Methods: In this study, 35 hemiplegic patients participated, with a mean age of 54.3 ± 17.9 years. The participants received resistance training for 18 weeks, twice per week, using lower extremity pneumatic resistance machines. Three main modes were used, namely horizontal leg press, knee extension, and knee curl. Muscle performance (strength, power), Balance (Berg balance scale [BBS], and computerized posturography), and body composition were measured. Results: We found a significant improvement in muscle strength and muscle power after resistance training (p < .01; p < .01), and a significant difference in BBS score after training (p < .05). However, no statistically significant difference was found in computerized posturography findings. BBS score positively correlated with lower extremity muscle strength in terms of leg press (r = .54, p = .007), knee extension (r = .56, p = .004), and knee curl (r = .49, p = .014), and with lower extremity muscle power in terms of leg press (r = .51, p = .002) and knee extension (r = .44, p = .009). No statistically significant difference was observed in knee curl. Conclusion: Lower extremity resistance training can improve balance in hemiplegic patients. Therefore, lower extremity resistance training can be considered as routine training option for hemiplegic patients. |
