| 中文摘要 | 尿失禁對婦女造成許多日常生活上的困擾而降低生活品質,其治療方式除了手術及藥物控制外,又以骨盆底肌肉訓練為首選。過去研究多著重在骨盆底肌肉強度及耐力的訓練,較忽略尿失禁的致病機轉,宜加入腹壓變化覺察、肌肉協調及間歇收放訓練,配合生理回饋儀的輔助,使婦女在訓練過程中透過視覺化方式強化訓練效果,將有助於改善漏尿問題而達到治療成效。本文旨在探討配合尿失禁致病機轉,並依美國運動醫學會運動處方設計原則FITT-VP,即頻率、強度、項目、時間、量、漸進等內容,規劃骨盆底肌肉訓練運動處方,期能提供醫護人員作為臨床訓練婦女執行骨盆底肌肉運動之參考,以提升照護品質。 |
| 英文摘要 | Regarding the methods for treating urinary incontinence, pelvic floor muscle training has become the most preferred method in addition to conventional surgery and medication. However, previous related studies have primarily focused on pelvic floor muscle training and endurance training while neglecting the pathogenesis of urinary incontinence. Devices for detecting abdominal pressure, muscle coordination, and intermittent retraction training should be combined with biofeedback instruments to provide visual information to the patient throughout the training process to improve training outcomes. Therefore, this article adapted the guideline of exercise prescription suggested by the American College of Sports Medicine, including frequency, intensity, type, time, volume and progression (FITT-VP). We applied FITT-VP to examine exercise-based treatments in the literature as well as the pathogenesis of urinary incontinence. A biofeedback-assisted pelvic floor muscle exercise training program was developed for use as reference in clinical treatment. |
