中文摘要 | 今年初的流感的疫情十分嚴重,發生許多流感併發重症。我們報告一位40歲男子,為白塞病(Behcet disease)患者,有酗酒習慣。他因水腫來門診就診,發現有腎衰竭。後來一系列檢查竟然是流感重症引發急性心肌炎合併心臟衰竭,進而引起腎衰竭,水腫是因心腎衰竭。儘管積極治療,病患仍發生呼吸衰竭和休克,使用體外心肺維生系統進行搶救。不幸的,病患仍然死亡。這病例提醒我們,在流感流行的季節,流感的症狀有時不是很明顯,特別是在免疫不全的病人,臨床醫師仍需想到有可能是流感。 |
英文摘要 | In the Spring of this year, the influenza epidemic in Taiwan was very serious, there were many severe complicated influenza. We reported a 40-year-old man who was a case of Behcet's disease and alcoholism. Due to edema and shortness of breath, he visited our Out-Patient-Department. Renal failure was noted. After admission, a series of studies revealed influenza A induced acute myocarditis with heart failure, thereby causing renal failure. Edema was due to heart failure and renal failure. Despite of aggressive treatment, respiratory failure and shock still developed. Extracorporeal membrane oxygenation system was performed. Unfortunately, the patient was dead. This case reminds us that in the influenza epidemic season, flu-like symptoms were sometimes not very obvious, especially in immunocompromised patients, but clinicians still rule in the of possibility of influenza. |
