| 中文摘要 | 目的:利用重要-表現分析,評估SNOMED對國際疾病分類第十版編碼的價值與意義。方法:於2015年10月至12月調查南部三家醫院339位員工對於SNOMED協助國際疾病分類第十版編碼的認知。利用現代測驗理論單參數Rasch(羅序;1960)模式,將原始分數轉換為等距的洛基分數,進行模式擬合度的統計檢測,圖示受試者的洛基得分盒形圖,繪製彩色人與題目對照圖,及繪製重要-表現分析的尤登圖與填卷者身份的性別人數金字塔圖。結果:結果顯示:(1)17道題目的同意度與重要性量表,皆呈單向度的測量(內部一致性信度分別為0.83及0.87)、(2)疾病分類人員及醫師表達對SNOMED較高的同意度與重要性、(3)94.1%(=16/17)題目呈現同意SNOMED的價值與意義。結論:引進許多國家採用的SNOMED轉換國際疾病分類第十版的編碼方式,期以資訊人員的努力,以提供醫師另一具填列ICD-10的選擇方式。 |
| 英文摘要 | Objectives: To evaluate perceptions about the Systematized Nomenclature of Medicine (SNOMED) in terms of International Classification of Disease version 10 (ICD -10) coding using Importance-Performance Analysis (IPA) Methods: We queried 339 employees from three hospitals in southern Taiwan regarding their perceptions about SNOMED in terms of ICD-10 coding from October to December 2015. The Rasch (1960) model based on item response theory (IRT) was utilized to transform ordered categorical scores into interval Rasch logit scores and then used to test the data-model fit. Visual representations using a box plot with a logit unit, a colorful item-person map, IPA Youden plot, and a pyramid plot with describing respondent count number across gentles were made. Results: The results showed that (1) both 17-item scales of agreement and importance were unidimensional, (2) the highest scores for perceptions about SNOMED were from respondents in terms of disease classification and from clinical physicians, (3) 94.1%(=16/17) of the items showed a positive attitude about SNOMED as used in hospitals. Conclusions: Use of the international SNOMED by computer programmers can help physicians select appropriate ICD-10 codes in clinical settings. |
