中文摘要 | 核磁共振影像能多平面顯示肩關節損傷,但掃描時間長,常受到患者各種不自主運動及疼痛難耐等多種因素,導致影像中出現假影,對分辨圖像中細微結構十分不利,嚴重地影響臨床診斷。因此我們提出使用螺旋槳採集序列( PROPELLER)來減低假影的產生,預其增加影像品質及提高檢查成功率。本研究分別使用常規 T2加權快速自旋回波脂肪抑制序列( FSE/T2WI/FS)和螺旋槳採集脂肪抑制序列(PROPELLER/T2WI/FS),於平穩無移動、呼吸造成假影、肩部移動三種模擬情況下進行掃描,由兩位觀察者對影像評分並行盲測分析及有效訊號雜訊比、對比雜訊比分析影像品質,並以統計分析差異。兩位觀察者影像評分平均值結果,兩種掃描序列在三種模擬中,四種影像(呼吸假影、運動假影、血管搏動假影、影像整體品質)之評分平均值,以成對母體平均數差異檢定(Paired T-test)分析 P值都小於 0.001,說明自旋回波及螺旋槳採集序列兩者在這些分析中有顯著性差異,使用螺旋槳採集序列影像可減少假影的產生使影像品質獲得改善。影像品質分析,分析影像有效訊號雜訊比,兩種掃描序列在三種模擬中,兩者序列 Paired T-test P值都小於 0.001,說明有顯著性差異,且其使用 T2加權快速自旋回波脂肪抑制序列有效訊號雜訊均較高。螺旋槳採集序列能夠顯著降低運動產生的假影,更好地顯示肩關節解剖結構細節,做為這研究良好結果,建議使用螺旋槳採集序列於常規肩關節核磁共振作為長 TR對比序列掃描,取代自旋回波序列。 |
英文摘要 | MRI provided multi-plane images for shoulder girdle injuries. However, due to prolong scanning time, motion artifacts were caused by involuntary movement or in-tolerate pain. These artifacts greatly affect the details of images and make it difficult for clinical diagnosis. PROPELLER sequence was expected to reduce artifacts and increase image quality. Routine T2-weighted image fast spin echo fat saturation was compared with PROPELLER T2-weighted fat saturation under three conditions: stable without motion, breathing motion and shoulder motions. Two observers rated the images under blind test and analyzed the image quality using effective signal to noise ratio and contrast signal to noise ratio with statistical analysis difference. The P-value of the mean values of two observers’ rating results and the results of two sequences under three conditions with four images, breathing artifact, motion artifact, artery pulsing artifacts, and total image quality, are both <0.001 with paired T-test. This implies that spin echo and PROPELLER are significantly different and using PROPELLER sequence can reduce artifact and improve image quality. Image quality analysis of effective signal to noise ratio in both sequence under three conditions have P-value smaller than 0.001. T2-weighted fast spin echo fat saturation sequence has a higher effective signal to noise ratio. PROPELLER sequence can significantly reduce motion artifact and provide better anatomical details of shoulder girdle. For these good characteristics, we favor the use of PROPELLER sequence routinely for shoulder girdle MRI with the long TR contrast sequence scan instead of the spin echo sequence. |
