中文摘要 | 口腔癌的病理組織切片中,在神經周圍的組織空隙或是神經外膜内發現口腔癌細胞就稱作口腔癌的神經侵犯。神經侵犯是一個重要的病理組織學特徵,研究具有這樣特徵的腫瘤容易增加區域復發的機會,有較差的預後及較短的存活時間。現今神經侵犯已經是口腔癌病理組織檢查中常規進行的項目。但神經侵犯的數目與存活之間的關係則鮮少被討論。本回溯性研究收集本院自1998年起進行口腔癌切除手術共計580名病患,由神經侵犯的數目探討與存活間的關係。研究結果發現有口腔癌神經侵犯的病患具有顯著降低的存活期間,並且侵犯神經數目大於3條的病患有更高的風險。在具有這樣病理組織學特徵的病患身上,使用更積極的輔助性治療,可能達到更好的治療效果。 |
英文摘要 | Perineural invasion (PNI) in oral squamous cell carcinoma (OSCC) indicated that the tumor cells invade into the surrounding spaces of nerves. It has been considered as an important pathological feature related to the prognosis and the survival rate. PNI was found not only in the advanced stages of OSCC, but also in the early stages of the disease. This pathological feature implies higher chance of tumor recurrence and poorer prognosis, as a result, PNI was examined routinely by the pathologist in our clinical setting. The relationship between the number of involved nerves and the outcome of OSCC was rarely discussed. The aim of this study was to explore the relationship between PNI and the survival time of OSCC. Material and Methods: We had reviewed the OSCC patients who had received tumor excision operation at our hospital. A total number of 580 patients were included. The presence of PNT and the survival time were recorded. The quantity of the involved nerves was also counted. Results: In univariate analysis, it showed a strong influence of PNI on the survival time (P<0.0001). Receiver operating characteristic curve (ROC curve was used and the best cut-off value of the affected nerves was set as 3. Conclusion: Perineural invasion is a predictor of poor prognosis of OSCC patient. Our study demonstrated that PNI more than 3 nerves was a poor prognostic factor of OSCC. |
