中文摘要 | 全球第一型糖尿病(type 1 diabetes mellitus, T1DM)青少年發生率持續上升,且青少年期血糖控制差,易增加糖尿病急慢性合併症的進展,導致高致死率及致病率。因此了解影響TIDM青少年糖化血色素(glycosylated haemoglobin, HbA1c)之研究實為重。許多國內外研究發現T1DM 青少年自主性是影響糖尿病自我照護的重要因素,進而與HbA1c 控制相關。本文旨在透過系統性文獻回顧整合青少年自主性對糖尿病控制之影響與成效,提出相關建議,為後續T1DM 青少年研究與臨床實務參考。綜合PubMed, CINHAL, ProQuest 及OVID 四個資料庫於2015年1 月前發表的文獻,鍵入自主、糖尿病、青少年等關鍵字,限量性研究文章,經篩選及刪除重複文獻後得16 篇文章符合標準。16 篇文章研究對象皆為第一型糖尿病患者,年齡範圍在10-19 歲。其研究目的著重於青少年自主對糖尿病控制的影響,以及青少年與照顧者互動相關性對青少年自主與糖尿病控制的影響。學者探討青少年糖尿病照護影響者以父母親角色佔最多(94%),顯示父母自主性支持影響對青少年自主、糖尿病控制及成效的重要性。整體顯示,青少年自主、糖尿病自我照護與HbA1c 控制呈正相關性。 |
英文摘要 | The incidence of type 1 diabetes mellitus (T1DM) has increased in the majority of regions worldwide. Poor control of glycosylated haemoglobin (HbA1c) during adolescence can increase the development of short- and long-term diabetes complication that cause high morbidity and mortality. Therefore, it is important to understand HbA1c control among T1DM adolescents. Numerous studies have confirmed that autonomy is a crucial factor of diabetes self-care related to HbA1c control in T1DM adolescents. In this study, we conducted a systematic review of studies that examined the effects of adolescent autonomy on diabetes control to facilitate the development of recommendations to support adolescent patients with T1DM. Experimental articles published before Jan. 2015 from the PubMed, CINAHL, ProQuest , and Ovid databases were searched with the keywords "autonomy," "diabetes," and "adolescent." After removing repetitive results, 16 studies that fulfilled the required criteria were identified. The participants in all 16 studies were adolescents with T1DM aged between 10 and 19 years. Studies focused on adolescent autonomy related to HbA1c control and the influence of caregiver-adolescent relationship toward adolescent autonomy and diabetes self-care. The majority of studies (94%) focused on parent-adolescent relationship. It showed that the influence of parent autonomy support for adolescent autonomy and diabetes control is important. Overall, adolescent autonomy, diabetes self-care, and HbA1c control were strongly correlated. |
