中文摘要 | 因急診與住院的病人於執行胸腔站立攝影時所發生之跌倒事件往往造成重大傷害,所以本研究加入使用安全固定帶做為固定病人站立攝影時的輔助工具,探討急診與住院病人使用安全固定帶是否影響影像判讀結果與應用於臨床胸腔站立攝影之可行性。研究中安全固定帶使用帆布材質與魔鬼氈,並於內層夾入發泡棉以改善結構形狀及提高支撐強度。所使用之X 光機為TOSHIBA MRAD-A50S,CR (computed radiography)與DR(digital radiography)影像讀取系統分別為:(1)FCR XG5000 CR(富士,日本)讀取影像。(2)CXDI-40EG DR(佳能,日本)讀取影像。安全固定帶下方結合鉛圍裙,直接覆蓋在PIXY 假體(Universal Medical,美國)胸腔下照射取得影像,再由五位5 年以上判讀經驗之放射線專科醫師進行影像判讀。CR 與DR 影像之分析結果P 值為0.020,代表有顯著差異,由此結果發現DR 影像品質優於CR。安全固定帶經過實驗影像之分析結果P 值為0.096,沒有顯著差異,此安全固定帶不會產生假影影響醫師診斷結果。所以使用安全固定帶於臨床輔助工具固定住病人,防止跌倒受傷事件發生,可大幅降低因跌倒產生之醫療糾紛。 |
英文摘要 | Patient falls due to taking stand-up chest X-rays of emergency patients and inpatients often cause serious injuries. This study was introduced to use a safety fixed belt as a auxiliary tool to help stabilize patients while taking stand-up X-rays and discussed whether the use of safety fixed belt in emergency patients and inpatients would affect the image interpretation or not and the feasibility of using safety fixed belts applied in taking stand-up chest X-rays. The safety fixed belt used in this study was made from canvas and Velcro. Foams were affixed to the inner layer of safety fixed belts to improve their structural shape and enhance support strength. Toshiba MRAD-A50S X-ray machine was used. The CR (computed radiography) and DR (digital radiography) image reading systems were FCR XG5000 (Fuji, Japan) CR and CXDI-40EG (Canon, Japan) DR, respectively. A lead apron combined below a safety fixed belt was directly covered PIXY phantom (Universal Medical, America) under clinical chest irradiated condition to obtain X-ray images. The X-ray images were then studied and compared with those taken without safety fixed belts by five radiation specialist physicians with more than 5 years of X-ray reading experience. P value was 0.020 and had a significant difference from the results of CR and DR images. The results showed that the quality of DR is better than CR. P value was 0.096 and had no significant difference with using safety fixed belts through the analysis of experimental images. Using safety fixed belts will not produce artifacts and has no influence on physician diagnosis. Therefore, the use of safety fixed belts as a clinical auxiliary tool immobilized on the patient can be used to prevent the injuries of patient falls events, and dramatically reduce the probability of the occurrence of medical disputes due to patient falls. |
