中文摘要 | 研究目的: 針對產後婦女,使用生化湯治療產後腹痛及惡露,研究臨床上的徵狀改善,並偵測其體內總體代謝物之變化,驗證服用生化湯對人體代謝的總體影響,其與臨床上的徵狀改善有無相關性?本計畫引入代謝體學(metabolomics)的觀念,利用偵測體內總體代謝物的圖譜,以體內總體代謝物的表現作為指標,在使用中藥方介入後有無改變? 研究方法: 一、針對產後婦女經評估產後可使用生化湯者。二、收集(一)實驗組:有腹痛及惡露組使用生化湯(使用前/後)之產後婦女 (二)控制組:無使用生化湯之產後婦女抽取收案時 (V1)及服藥五天後(V2) 血液做為檢體。三、進行臨床療效評估:包括疼痛視覺量表、惡露日誌等。 四、進行血液生化值分析,並以 LC/MS/MS偵測檢體中之總代謝物。五、以 MarkerView軟體分析中藥介入,對體內總體代謝之影響及臨床療效之關連性。 結果與討論: 一、中藥方劑生化湯指標成份定量分析 以 HPLC分析本實驗所使用之生化湯(GMP廠產品玉女生化湯,為規格化藥湯包),以煎煮法測得每包指標成份 Ferulic acid僅為傳統生化湯飲片每劑的 0.25倍。故本實驗給予產婦 4包生化湯湯包。 二、篩選產後有腹痛、惡露現象之產婦,給予生化湯 5日。納入試驗者有 56位,生化湯組 27人實際完成二次抽血者為 24人。控制組 29人,實際完成者為 25人。 三、檢測血液中生化值分析:結果顯示這些生化指標的基礎值並無顯著的差異 (p>0.05)。服用生化湯後肝功能指數有些微下降,不過皆在正常值範圍之內,對肝功能並無影響。 四、臨床療效評估:服用生化湯組對產後宮縮視覺疼痛有明顯的改善(p=5.755E08)。但是乳脹疼痛、痔瘡疼痛、宮縮改變程度與控制組無明顯差異 (p>0.05)。對於傷口疼痛和惡露量的改善亦未比控制組好。 五、檢測血液中代謝物圖譜:以 ESI LC/MS/MS來檢測服用生化湯的血液檢體的總體代謝物圖譜。服用生化湯前 /後、服用生化湯組 /控制組,以 PCA分析總體代謝物,皆落在相同區域無法分羣。顯示總代謝物變化未受服用生化湯影響。 |
英文摘要 | Aim: To study the relief of abdominal cramp by Sheng Hua Tang in postpartum women. Therapeutic effect and changes in total metabolites were detected. Correlation was analyzed between changes in total metabolites and clinical improvements. The concept of metabolomics was introduced in this project to detect total metabolites spectra as biomarkers of Chinese herbs intervention. Method: 1. A 2-arm trial will be performed in postpartum women with abdominal cramp. 2. Collect blood samples of the two groups (1) Sheng Hua Tang (before/after treatment) (2) postpartum control without treatment. 3. Evaluation of clinical use includes visual pain score and bleeding diary. 4. Total metabolites detection using LC/MS/MS. Marker View software was employed for analysis of correlation between metabolites changes and clinical benefits. Results & Discussion: 1. Quantitative analysis of Sheng Hua Tang via marker component Sheng Hua Tang bags were extracted by boiling water. Ferulic acid as marker component was detected only 0.25 fold of the traditional prescription using HPLC. The dose was four bags of Sheng Hua Tang in this project. 2. There are 56 persons enrolled. Test group was 27 patients and 24 patients complete the trial. Control group was 29 patients and 25 patients finished the trial. 3. Biochemical data analysis Baseline of the biochemical data was not statistically significant in the two groups. Although SGPT slightly reduced after Sheng Hua Tang treatment in test group, the changes of the titers was irrelevant to hepatic function. 4. Clinical improvement Uterus constriction was significantly improved in women treated with Sheng Hua Tang. Other types of pain were not attenuated. 5. Detection of plasma total metabolites Only minor differences of total metabolites were detected in treated group using ESI LC/MS/MS. |
