| 中文摘要 | 植牙治療已經廣泛用於重建缺牙區的功能及美觀,雖然目前植牙治療的存活率已高於90%,然而植牙的存活率則會受到許多因素影響,其中過重的咬合負荷則可能是造成機械性植體併發症的原因之一。由於骨整合的植體無牙周韌帶,植體因此可能較易受到咬合過重負荷的影響而導致周圍的骨喪失,進而影響植體的預後。本文回顧動物及臨床研究文獻探討植體過重負荷對於骨喪失及對植體周圍組織的影響,發現在臨床研究尚無足夠的證據顯示過重負荷對於骨喪失或植體喪失有因果關係,在動物實驗則顯示在無發炎的環境下,過重負荷對於骨整合並沒有負面影響甚至增加骨合成;但若在發炎的狀況下,則會明顯的加強牙菌斑引起的骨吸收。因此,臨床上在植牙治療中,應瞭解並注意整體咬合負荷及植體清潔問題,才可訂定適當的治療計劃,減少併發症的產生,並提高植牙治療的預後。 |
| 英文摘要 | Implant treatment has contributed to restoring oral function and esthetics to many patients who have reduced dentition. Reported implant survival rates are greater than 90%. However, implant survival rates appear to be related to multiple factors. Occlusal overload may be one causative factor associated with mechanical complications. Osseointegrated implants lack the periodontal ligament and therefore may be more prone to occlusal overloading, which is regarded as one of the potential causes for peri-implant bone loss and thereafter affects the prognosis of dental implants. The aim of the present article is to evaluate and investigate the effect of implant overload on peri-implant tissue health through a review of animal and clinical studies. Clinically, these articles provide little unbiased evidence supporting the idea that there is a cause-effect relation between overload and bone/implant loss. At the animal level, overload when present in an uninflamed peri-implant environment, does not seem to negatively affect osseointegration, and can even be anabolic. In contrast, overload in the presence of inflammation does seem to significantly increase plaque-induced bone resorption. Thus, during the dental implant treatment, clinicians should pay attention to occlusal loading and oral hygiene care, which will involve making an appropriate treatment plan that reduce the adverse effects of such factors on implants Such an approach should increase the predictability of implants. |
