中文摘要 | 當病患有多的捐贈齒提供時,利用自體齒移植手術來重建缺牙區是可行且有效的方式,維護移植捐贈齒牙周韌帶的活性完整,是達到移植成功的最重要因素,也因此,必須盡可能減少移植齒在口外暴露的間。本病例呈現上顎後牙缺失且上顎竇較低的病患,欲以多餘智齒進行自體齒移植,並合併上顎竇提升術來達到上顎後牙區重建。過程中用錐狀射束電腦斷層掃描做詳細的診斷與分析,製作電腦輔助擬真齒和擬真受植區模型,在口外模擬手術操作狀況,決定受植區需修形的大小和移植齒放入的方位,更於實際牙齒移植時,以擬真齒模型來試戴,以縮短移植齒口外暴露時間,減少牙周韌帶細胞不必要的傷害,此外還能達到移植齒和受植區的理想接觸及初期穩定,增加自體齒移植的成功率。 |
英文摘要 | Autotransplantation of teeth is a viable option when replacing missing teeth if a donor tooth is available. The maintenance of a healthy periodontal ligament of the donor tooth is a critical factor for successful tooth transplantation. Achieving this depends on minimizing the extra-oral time during the surgical procedure. The patient suffered from a missing maxillary molar and had a pneumatization of the maxillary sinus in this area. The restorative treatment we performed was autotransplantation of the third molar combined with sinus floor elevation rather than implant installation. Cone-beam computed tomography was carried out in order to obtain a comprehensive analysis of the patient's anatomy and the dimensional data was used to fabricate computer-aided prototyping models of the donor tooth and of the recipient bed. These models allowed pre-contouring of the recipient site and helped to decide the proper location of donor tooth. Together these shortened the extra-oral operation time and minimized unnecessary damage to the periodontal ligament. These improvements resulted from that these models could be checked against the prepared socket rather the donor tooth. In addition, these models helped enhance optimal contact between the donor tooth and the recipient site, as well as improve the initial stability of the donor tooth. |
